Historical change record. The September 11 application-wide shutdown and its blanket resumption restrictions were superseded by the operator on September 15. This page records past actions and results; it does not impose current execution restrictions. See the stop-scope recovery record.
The operator requested improving the existing review agent and validating exactly one issue categorization before deciding whether to allow more. Selected native issue: AutoCoder/AutoCoder-Engine #1569, excessive repeated history on #1456. This does not authorize implementation of #1569, manual recategorization UI work, or a general automation restart. Final operator assessment remains pending.
PR4372, candidate 4aa9a8af6909f8876af5849bb796629dabf6f530, adds an embedded independent-review evidence contract and a categorization-only worker path. Requirements checkpoint 3ec07cd9 preceded implementation. The reviewer must distinguish original intent, introducing change, observable rationale, RCA gaps, test relevance and remaining live evidence. One exact admitted native issue is assessed in a read-only sandbox without managed-system MCP tools; the engine validates the unchanged original row and writes classification plus readable rationale with assessment provenance. Operator corrections and unrelated records are protected. No implementation or successor task is admitted.
Verified context kubernetes-admin@kubernetes at 192.168.1.9:6443. Backend remains server-only and frontend remains online. Gitea Actions disabled. Native issue1569 remains uncategorized before the trial. Current source evidence confirms 35 feedback-accepted and 35 feedback-source-migrated entries on September9 plus three other history records; no fresh recurrence is inferred. Unrelated issue fingerprint: 273 rows, e29e267ff80332cece786be37e1fc932.
1,778 native tests passed, 93 ignored; one isolated PostgreSQL test separately passed for scoped persistence, wrong worker, stale evidence, repeated application, operator-priority preservation and unchanged unrelated rows. Formatting passed. Disposable PostgreSQL removed. The first disposable Docker bridge port was unreachable; a bounded loopback host-network test container worked. No production database was used for these regression tests.
Exact immutable image build is in progress. Merge, deployment, worker admission, category persistence and browser verification are not yet claimed. The planned supervised pod has no service-account token or host mounts, bounded local ephemeral storage and memory-backed credentials. Existing account/model admission and fallback reserve checks remain active; no paid API use is authorized. The supervisor will retain bounded credential-free evidence and remove the pod after execution.
Artifacts: /home/ax_julian/temp/autocoder-categorization-review, bounded256MiB; current administrator owns retention review by September13. The source/cache worktree remains the existing bounded task workspace. General automation stays stopped throughout.
PR4372 merged as 7e9c21f59e28d9f9f310ef35630bb1fff4b5ffaa. Backend image e2e5ac6739daecfa4a424302168b4ddab8106404b3a5dd0fe4ceaea48488da0d is deployed ready in server-only mode, retaining the actual frontend. The independent reviewer instructions are now embedded in future verification prompts; this does not itself prove reviewer effectiveness on future incidents.
The first supervised invocation stopped before claim because normal storage admission required 200GiB free; the worker node had95GiB. No model execution or category change occurred. Requirements checkpoint f1500d93 and PR4373 fixed only supervised read-only assessment admission:4GiB free for the3GiB ephemeral bound plus headroom, retaining maximum-used and all other task-kind safeguards. Full1779 tests passed,93 ignored. PR4373 merged84a51f655a4ece96616ef5af05526874e6cdd2c2; its exact built worker image861c87cae7983fbca39f0b2807ce9a994e2a9bd86d2b626271b73312fdca7c6d ran the same retained task, without re-admission or reset.
Task issue-categorization-1569-27d5ecb78864a53da8420baa770d52b831a85e00ff6047780668278ffaa09f6b completed with16437 recorded tokens on gpt-6-astra/high. Account refresh reported66percent used; the30percent manual-only fallback reserve remained protected. No paid API route was used. The model assessed issue1569 as bug, important=false, urgent=false, complexity=unassessed. It cited historical duplicate rows and lack of established wider impact or fresh recurrence; these are assessment reasons, not operator approval or proof that wider impact is absent.
The engine finalizer applied exactly one classification and one worker-categorized history entry with assessment provenance, keeping status review_required. All273 unrelated issue rows retained fingerprint e29e267ff80332cece786be37e1fc932. All6297 earlier tasks retained fingerprint2989d79603d52688368e16be4a3b8b2a. No queued/running tasks or valid online/busy worker leases remain.
Actual Chromium verification at https://codecartography.dev/issues found issue1569 in Not Important + Not Urgent, displayed its rationale, and preserved both priority values on Refresh. No JavaScript errors; work-starting and chat routes remain503. This used a temporary session for the independently verified allowlisted administrator actor, revoked afterward; it does not claim testing interactive OAuth or personal operator acceptance.
Visual inspection caught a material gap omitted by the narrower priority assertions: the model and raw database retain complexity=unassessed, but the actual UI displays Simple. Source inspection identifies normalize_complexity in src/codecartography_issue_console_helpers.rs defaulting unsupported values to simple. Therefore this trial proves scoped execution/persistence and visible priority, but does NOT pass complete classification acceptance. No claim is made that the assessment's importance judgment is correct; missing demonstrated impact needs scrutiny rather than automatic acceptance. Recommendation: do not expand categorization yet. No additional issue was created or existing issue closed for this discovery.
The supervised pod, disposable test database and build containers are removed. Browser credentials/session were revoked; no debug page was deployed. The source prompt, original issue snapshot, result, model, thread identity, runtime image, accounting, screenshot and state comparisons are retained in the existing bounded local evidence directory (668KiB at completion;256MiB bound; reviewSeptember13). The same prompt/result/accounting remain durable in CodeCartography. General automation and Actions stay stopped; final operator evaluation remains pending.
The operator rejected the issue1569 pilot: low priority was unsupported, complexity contradicted the stored unassessed value, and historical administrator instructions had been misplaced in the active body. Codex authored that historical recording-only paragraph; attacker authorship is not established. The worker cited deferral, demonstrating contaminated evidence but not hidden decision causality.
On the operator's explicit approval, the administrator atomically corrected only native issue1569: removed the historical restrictions from its active description, recorded operator-important=true and urgent=true, retained complexity=unassessed and lifecycle state, added a red instruction-contaminated-assessment-rejected label, and preserved the complete original row and prior assessment task in an assessment-rejected audit event. No Gitea issue changed and no restart occurred. The flag is visible evidence, not an execution gate.
Read-only findings: issue1456 has 73 history events, including35 copies of each of two handoff messages for feedback41821 between19:55 and22:50 UTC on9September. One linked cycle96629 failed during planning; no implementation or verification task is linked. Its blocked planning task records812544 tokens; these are not attributable to the duplicate history without further tracing. Current writers append both messages unconditionally on repeated successful ingestion/recovery, but current candidate selection excludes existing native feedback. The September9 deployed call path and reason for repeated entry remain unverified. No new repetitions on this issue are observed; emergency stop makes that insufficient evidence of a fix.
Requirements checkpoint73b623f3 on fix/issue-assessment-evidence requires undecided evidence gaps, isolation of problem text from execution authority, operator-protected reassessment and truthful complexity display. Implementation/testing and a single proposal-only reassessment are in progress; no acceptance or loop-repair claim.
At08:40:23 UTC the administrator ran an unbounded concatenation of complete worker-task JSON rows to fingerprint existing tasks. PostgreSQL killed that query process with signal9, terminated other connections and recovered by08:40:24. The deployment had no pod restart, but the backend retained closed database connections. The administrator restored the same stopped server-only backend by a guarded template annotation rollout, then verified an authenticated issue-console API request successfully read issue1569 with corrected operator priority and red flag. The old backend still projected unassessed as simple until the planned replacement. This interruption was caused during this administrator verification, not by an issue worker. Future checks use per-row bounded hashes or only task lifecycle columns with a ten-second statement timeout; no aggregate of complete retained prompts/results. No data loss is observed; the corrected issue was re-read. The simple health endpoint alone was insufficient to prove database-dependent application recovery.
PR4374 merged as63b4a1ea15213fda733ed4f37dc5e60809c0a975; candidate7d65d276697a deployed as immutable image7b67ce88f05307b41fbeabef42cd000fe3f628a60656d209e71fbfcc333f3ec1 with server-only arguments. Browser navigation to the actual issue card verified Important Yes/Urgent Yes and Complexity Unassessed with the rejection flag and cleaned active request. No reassessment proposal was present, correctly failing the complete browser acceptance check.
Task issue-categorization-1569-f2994504c8cd318ac418ecdfcbbab9908de0819e879c927f2871fc83ce8ccefa failed without changing the issue: result says it could not read .runtime/tmp/task-instructions.md because bwrap could not create a namespace. Recorded usage66237 tokens is retained; no successful assessment is claimed. An earlier pre-claim invocation lacked the required worker database URL; corrected from the pod's existing Secret without exposing it. The subsequent model attempt used a fresh provider capacity reading of1 percent used, preserving the fallback30 percent reserve.
Confirmed prompt-delivery cause: the shared codex_execution_prompt helper (file-deferral introduced in4dc3afb78,28August) returns a file reference above4000 characters. The newly introduced supplied-evidence-only assessment inherited that tool-dependent path. Its full prompt was stored but not delivered directly. Requirement2223e40d and implementationada63d2e under PR4375 make this task kind receive complete evidence over stdin; sandbox permissions remain unchanged.1780 tests passed,93ignored, including the extended delivery regression. An authorized retry of the same assessment outcome awaits its tested image; original task/result/accounting remain intact. Temporary failed worker pod was removed.
PR4375 merged90fa4e1c517ed23d8b5ccf801f8a936620d2a615; backend and supervised worker used image3a969aa23e77125e9c3906ce41c862c26a35964daafedcbcee48285a8a466c86 (candidateada63d2e). Together with the failed file-delivery attempt, this correction's review attempts consumed84330 recorded tokens; no paid API route was used. The earlier categorization pilot's16437 tokens remain separately recorded.
Result: important=true, urgent=true, complexity=unassessed, explicitly preserving authoritative operator priority. Rationale recognizes the damaged audit trail, refuses to treat unknown harm or quiescence under stop as low priority, and leaves material gaps for September9 deployment/call tracing, duplicate execution/resource attribution, affected scope, recurrence conditions, contaminated historical conclusions and repair validation. This is a review of supplied administrator investigation evidence, not an independent live model investigation or a fix to repeated handoffs. No further classification or implementation was admitted.
Bounded before/after evidence: all274 issue rows unchanged by both review attempts (per-row-hash aggregate1904611694f7408a1106d2c88c235bc0); all6298 preexisting task lifecycle rows unchanged (d5d9bcfe64d1493dd06d2aa0fcfb2c28). Zero queued/running tasks and zero valid online/busy leases after completion. Actual Chromium application review passed refreshed priority/complexity, cleaned active description, preserved audit, proposal presence, authentication/logout and503 work-start boundaries, with no JavaScript errors.
Visual acceptance found the label color was ignored by the shared card renderer, so the warning appeared as a gray chip. Requirements647ebf20 and98df098a and current branch fix/visible-issue-warning address validated label color and individually expandable audit entries. This also prevents preserving the original JSON evidence from overwhelming the active card. Frontend verification/deployment remains in progress; no claim yet that this visual correction is live.
PR4376 merged a83f8109062cd9fceb13551b524a12f3759a5b7b. Frontend source2a6c590055fb built successfully into image8e888c18ca3c69a943fa2283de5aa70e21543346831d0dca02bd9acd16e60ff2; aa38db90 pins this immutable image in the server-only overlay. Shared issue labels now render validated six-digit colors as a colored border and subtle background, retaining text contrast; audit events use native details/summary disclosures. The issue has no manual recategorization UI added.
Actual application browser acceptance passed after deployment: issue1569 remains Important Yes/Urgent Yes/Complexity Unassessed after refresh; active request excludes the misplaced paragraph; the red warning has computed border rgb(215,58,74) at4px and matching translucent background, while ordinary bug labels retain their blue color. Historical records start collapsed, the original removed paragraph remains inspectable under Assessment-rejected, and Worker-reassessment-proposed opens the new evidence and gaps. The warning remains visible at390px viewport width. No JavaScript errors; authenticated session/logout/revocation verified. Work-start routes return503, backend args remain server-only. A browser selector was updated to target the outer card summary after adding nested history summaries; no product defect was inferred from that stale test selector.
Backend image remains3a969aa23e77125e9c3906ce41c862c26a35964daafedcbcee48285a8a466c86, frontend image8e888c18ca3c69a943fa2283de5aa70e21543346831d0dca02bd9acd16e60ff2; both deployments available1/1. Zero queued/running worker tasks and zero active leases. Temporary reassessment/browser pod deletion confirmed, disposable database and isolated builders removed, temporary registry credential directory absent, own verification sessions revoked. Worktree clean; all code integrated. Evidence2.8MiB remains within256MiB parent bound, operator acceptance pending and retention review13September. Final screenshots and structured browser/model evidence are under /home/ax_julian/temp/autocoder-categorization-review/correction.
Acceptance requested is limited to corrected issue assessment, preservation of operator priority, honest evidence gaps and visible warning/history presentation. The underlying repeated-handoff issue is not fixed; normal autonomous pickup, implementation and resolution remain unverified under emergency stop. Do not admit further categorization or implementation until the operator decides the next scope. The administrator-caused query interruption above remains part of this record and must not be omitted from the outcome report.
The operator accepted the corrected assessment and presentation, then reported that the remaining red instruction-contamination label misleadingly implied a current defect. The administrator corrected only native issue1569's label to neutral gray Historical instruction contamination — corrected, preserving its identity, priority, complexity, problem description and previous audit evidence. A single assessment-warning-corrected activity records the former label and why it was superseded. This follows the operator's objection to the misleading warning; it is not a new finding of malicious injection. The underlying repeated-handoff investigation remains unresolved and no restart or worker admission was performed.
The operator authorized investigation and a repair proposal, not implementation or a worker restart. The application investigation and executable disposable-SQL fixture are now canonical repository documentation, merged in PR4377 as78b90e23c485e52e1fe0ae1a363d1d28100d836d: Repeated native feedback handoff.
Root mechanism recovered: b8bff771 (cycle62709, PR2928) explicitly attempted recovery of existing feedback without a cycle by allowing it through native intake again. The independent review approved that mechanism; the added test inspected source strings rather than successive passes while a cycle was delayed. Historical account persistence reset mirrored_at, explaining the roughly five-minute replay interval. Feedback41821 had35 calls/70 repeated action rows before cycle96629; its case occurrence counter also reached35. Another ordinary repository cycle96346 ended19seconds before96629 was admitted, supporting a serialization wait explanation; no per-decision admission trace is retained. Selected records show one planner, starting after repetition ended, not35 model executions. Resource costs beyond database/case/history churn are unproven.
Bounded same-repository September3–10 check found55 affected issues and55374 excess identical copies of the two handoff messages. Selector correction89cacabd was already deployed and is in current sourceada63d2e/image3a969aa. Disposable PostgreSQL reproduction with exact historical/current predicates produced70 versus2 activity rows over35 passes, one feedback in each; introducing a cycle stopped the historical replay. A stale selected-candidate replay still produced a second pair, demonstrating the remaining write-level gap as a fixture, not a proven production race.
Proposed next repair: make logical native intake, occurrence accounting and handoff audit publication idempotent under retries/concurrency/crash recovery while preserving genuine new source observations and all history. Test the real handler independently from candidate selection. This proposal was not implemented. No issue priorities, labels, lifecycle state or workers were changed by this investigation. Documentation-only merge did not run Actions (disabled); backend remains server-only and available1/1; active worker-task count0. Disposable PostgreSQL container removed. Small investigation artifacts retained within10MiB, review date14September; report/fixture are committed so reproduction is not dependent on temporary artifacts.
Following operator approval, PR4378 integrated repeat-safe native receipt/case/audit writes and was deployed under the existing emergency stop. A bounded same-image verification replayed production feedback41821 thirty-five times; receipt/case/issue fingerprints, the historical occurrence count and handoff history stayed unchanged, and no work was admitted. The backend and authenticated application are healthy; the workflow-start API remains503 and zero queued/running workers remain. Temporary verification resources were removed. Operator acceptance and normal autonomous delivery are not inferred. Full deployment and verification record.
