Historical change record. The September 11 application-wide shutdown and its blanket resumption restrictions were superseded by the operator on September 15. This page records past actions and results; it does not impose current execution restrictions. See the stop-scope recovery record.
The operator explicitly selected existing TheWell feedback41770 and requested tracing it through the engine. Expected product behavior: the first nonempty input line becomes the feedback title; the remaining lines form the description without concatenation or duplicated title. Preserve original development feedback identity b35d7200-5e95-4f67-8bae-50cb790f812a, case146368, and enrolled frontend repositorybc0e18b7-8626-49c5-8db3-d3ab2c8e0210. Reassess actual component ownership from source. No new feedback/issue is substituted. User correctness acceptance remains pending. General automation and Gitea Actions stay stopped. No production promotion of TheWell is authorized.
Current stored state: accepted, no linked implementation, requirements attempts0. Azure provider access works. Frontend dev revision366fe2819b9057d8684eeb5a0b7ec010a7a849c5 has successful pipeline25701; this does not prove reported behavior. The saved TheWell OpenShift kubeconfig is rejected with authentication required; no credentials were replaced. Actual live reproduction remains unverified.
Existing provider importer batches8 reports and cannot enforce this selected scope unchanged. Requirements checkpointf8a294ef and code9a06aae4 add supervise-feedback-once preview/admission through the same importer, retaining original feedback identity, accepted-state check, enrollment, writabledev, budget, capacity, retry and conflict gates. Selected SQL filters before LIMIT and rejects rejected/missing/wrong-repository/already-owned records without selecting neighbors. Existing ordinary batch behavior remains. Requirements writing gets the existing confined Git permission profile. Optional repository-managed k8s/security/supervised-provider-access.json references the existing credential registry Secret without embedding credential values or Kubernetes administrator access.
PR4386 mergedc3351cb00f5d9f9215df2fbaea4efff9ba044eae. Validation:1781 regular tests passed; new isolated PostgreSQL selection test passed in both lib/main test binaries. Rust formatting and diff checks passed. Worker image build is in progress; no TheWell task has been admitted yet. Backend remainsserver-only on existingimage2c2ef233.
Administrator Codex owns continuation and cleanup. Build uses isolated4CPU/24GiBbuilder with18GiBtmpfs and restricted ephemeral registry credential cleanup. Evidence directory /home/ax_julian/temp/thewell-feedback-41770 is bounded256MiB, review2026-09-15. Original1571 implementation/session remain preserved and paused; it has not completed independent engine review or delivery.
Next: finish immutable worker image, prepare exact supervised provider access, inspect/admit only41770, run requirements import and follow its preserved engine-owned successor. Reproduction, implementation, independent review, development delivery and user acceptance must be separately evidenced.
Read-only admission tracing found feedback41754 exhausted two imports in tasksuperproject-feedback-thewell-frontend-1788947472255301067, failed2026-09-09 with requirement line23 having a temporary status. The repository-wide retry guard therefore excluded all newer frontend reports. Current frontenddev366fe281 has24 requirements, all with valid fully_implemented_complete status; this does not establish product correctness. Requirements checkpoint922f2325 and code46db10fc (PR4387 mergeded04b055) retain the old terminal report and normal batch recovery gate, but allow explicit fresh selection after validating the exact current artifact. The selected feedback's own retry limit and active member-task gate remain.1781regular tests and extended isolated PostgreSQL cases passed, including retained exhausted attempts and unchanged normal batching. The first worker image19dcf96e was built but not deployed; corrected image is building.
The engine's existing Azure service principal successfully read the exact frontenddev ref. No provider credential was created or rotated. No TheWell task has been admitted at this checkpoint.
Final worker source46db10fce3e4 is imagesha256:02326b0a68befbd9c2d51e010ac4c2eae69bd8a0e3c7b117dde3c6158eb06445. Namespace quota rejected a second4.25CPU supervised pod; no quota was raised. The old1571 task was confirmedcompleted with no running tasks. Its sleeping pod and disposable PostgreSQL sidecar were removed after source/session/test evidence preservation; sharedhostPath5.8GiB/24GiB was retained. Podautocoder-supervised-41770 is now2/2Ready with the exact image, same confinement and existing provider registrySecret reference. No backend or TheWell production rollout occurred. The first retry while oldpodterminated was stillquota-rejected; creation succeeded after ordinaryterminationreleasedquota.

The old exhausted task was not copied into its recovery prompt. Exactworker-once execution is active (administrator terminalsession94910), and normal DBstate isrunning. Scope is requirements only, not application implementation. The importer log's legacy provider=paid label denotes the non-Phoenix path; actual selectedauthisChatGPTsubscription, and noAPIkey/paidroutewasenabled.

The original requirements task completed and the provider finalizer published frontend dev commit b4e910a0519cbd16c06ef54de437922c6db75a76. REQ-0002 now retains the original feedback reference and explicitly requires first nonempty line as title exactly once, remaining lines as description in original order. New behavior test T-REQ-0002-03 is planned, not passed; no product code changed in this stage.
The stopped scheduler exposed a missing scoped continuation. Code 9bd13ab77e4c4dc4962b86e49738984f94f09492, PR4388 merged 4afba292b62ba240b28d3de61b4871217fa5eccf, adds exact continue-feedback-once through ordinary provider admission, preserving rejected/unrelated cases and existing active-owner, budget, account and delivery-pipeline guards. 1781 regular tests and isolated PostgreSQL selected-link/rejection tests passed. Sleeping supervised pod worker image updated to exact sha256:28b53a394f00da4c2674495a63bbfe4ba462b0b66c5097be5a01d0f0b70a0c0b; running image verified after restart. An early read-only preview reached the old image and rejected the unknown command without starting automation; the subsequent preview on the exact new image succeeded.
At 14:04 UTC the engine admitted implementation task superproject-requirements-1789394665775441577 for the same original feedback and REQ-0002. Implementation is queued, not delivered. General automation remains stopped. The provider delivery function named reviewed merges does not itself demonstrate an independent agent review; that evidence must be checked before development merge. Host HTTPS verification currently fails local issuer trust and the saved OpenShift credential is rejected; neither was bypassed. Current user access to development chat has been requested for final acceptance. Port-forward session93376 serves only disposable test PostgreSQL; the old forward81253 was explicitly terminated.
The exact implementation task ran in thread 01a0a03c-bf92-7b12-a256-46375421e854. Its three-file frontend diff implements multiline extraction at the shared feedback normalizer/stream caller, adds regression coverage, and updates REQ-0002 test evidence. The worker reports focused requirement/artifact tests 18/18, lint, TypeScript/build, and bounded one-worker suite274/274 passed, excluding the unchanged origin/dev ChatModelSelector expectation failure. Root independently verified that component and test have no diff from origin/dev. Parallel Mermaid timeouts cleared in bounded execution. This is not an all-green unqualified suite.
At14:15:32UTC the real engine rejected publication: Requirement REQ-0002 must retain new bounded provenance for new or materially revised intent. Task is blocked and owning change set failed; no product commit/PR/deployment was published. The requirement diff changed only T-REQ-0002-03 test fields: status/evidence plus new ai_enabled:true. The latter is part of the governed acceptance contract, with omitted values displayed as false. Root has not weakened the provenance guard or edited the product implementation. The engine saved exact repair evidence in the excluded task evidence file, but its resumed agent said the prior goal was complete and did not repair that gap. The automatic repair allowance is consumed and preserved.
Root is repairing delivery of retained validator evidence into resumed execution instructions, plus an explicit original-task resume preserving thread, counters, prior failure and active member conflict checks. No general restart. The independent scoped delivery/acceptance-hold repair PR4389 merged a6c7566c7beabafdce9e8f92881b1d2b58b0c8fc, image6e766bd1f1a92eafc5bde044c3d93f331b708825d23f82dc858c7f5375a8c35e built but not deployed. Further recovery validation is in progress.
User confirmed current development-chat access. Root fetched the actual development frontend over verified HTTPS after obtaining the public Thawte intermediate from certificate AIA and verifying it against existing system trust roots; no TLS bypass or system trust mutation. Current served entrypoint before delivery is /assets/index-BB5AX4jF.js. Saved OpenShift credentials remain rejected and unchanged. Final behavioral verification and operator acceptance remain pending.
PR4390/source fd124e58ee5e merged f42ceb9c4c61d6d2d23449a0c5a10255737897ce. Resumed implementation instructions now receive bounded latest runtime rejection evidence while retaining existing authority and unchanged native goal. Exact resume-provider-artifact-once restores only the blocked original task and failed owning change set, retaining its prior error, thread, counters and consumed automatic-repair marker; conflicting active member ownership or changed task state prevents admission. Tests:1782 regular passed; isolated PostgreSQL checks passed in both binaries for conflict/idempotence/original accounting/unchanged unrelated task and exclusion of operator-acceptance waits from delivery timeout. Acceptance hold now has explicit command_state and no stall clock. Image build is in progress; no task resumed yet.
Accounting audit before resume: task row total236092 covers only the final automatic-repair segment. Retained token_usage_record entries contain43 unique response IDs, summing2425512 processed tokens (input2411960, cached2336512, output13552), equal to the retained cumulative thread record. Exact JSON and scoped pre-recovery patch are preserved under artifact-recovery in the bounded task evidence directory. This is an observed accounting discrepancy; the task row is not a truthful cumulative total yet. No paid API use was enabled, and live provider capacity still gates execution.
Source fd124e58 image c814e8a4f0585b51ab975821c5d49995ad687b3711daf4a4dcde4a09c672c268 was verified running. Original task resumed in the same thread at14:31UTC, but no new validator-context developer message appeared in its retained history. The agent locally committed the preserved frontend work as53ddb2b and again reported completion; runtime blocked the same provenance failure at14:32:22UTC. No product publication occurred. The first recovery fix is therefore not accepted as effective.
Official App Server documentation https://learn.chatgpt.com/docs/app-server and the installed0.153.4 generated schema identify thread/inject_items as the explicit model-history append. A disposable protocol fixture 01a0a057-5405-7e31-aa27-4cc8a018b5e6 verified acknowledgment and persisted attributed developer message with no model turn. Source 84f0d57a, PR4391 merged 0b50a1964a09d28c1f77ef1b9eb18b0f4613c490, replaces the ineffective resume-configuration evidence route with this history append, waits for acknowledgment, then reads/reactivates the unchanged goal. The runtime message is explicitly engine validator feedback, not operator direction or new authority.1782 regular tests, formatting and diff checks passed. New image is building in administrator session72620; original workspace/thread/consumed automatic-repair marker remain retained.
Azure dev pipeline25709 succeeded for requirements-only b4e910a; that is not product-fix delivery. Current next action remains original-task recovery, exact implementation review, governed dev delivery and pending operator behavioral acceptance.
The14:42UTC run on image637d017b demonstrated actual message persistence, but source84f0d57a reused request ID5 already assigned to account/read. The dispatcher treated every history acknowledgment as account authentication, repeating thread resume and context append. Its handshake timeout was only checked in an idle select branch and was starved by continuous output.4423 repeated diagnostic items accumulated in the original local thread (6,644,722-byte rollout); no new model turn or product publication started. Root's first process match used the wrong argv0, so it did not stop the process; root corrected the match, terminated worker29, then killed orphan Codex254/MCP485/git229 when SIGTERM was insufficient. Process inspection verified only sleep and nonexecuting zombies. Root corrected an earlier commentary claim that the timeout had stopped it.
Exact containment transaction restored only original taskblocked/change-setfailed and marked stopped lease supervised-1571-once-29-1789396950795124483 offline. Original thread history was preserved, not rewritten. Verified full-thread usage2915245 (input2900842,cached2821248,output14403) was reconciled into the task row; settled effective usage93997 retained. Detailed before/after evidence is in history-handoff/interrupted-task.json and containment-accounting.log. This is a local supervised-runtime failure; no product fix has been delivered.
Correction source04ea0e89, PR4392, assigns history append distinct ID6 and checks the deadline before every select iteration. An explicit --fresh-thread continuation retains the same task and implementation, original history/result-thread reference, settled usage and validator evidence, while consuming the existing one-fresh-thread allowance. Fresh execution gets the same attributed validator message before goal activation, without replaying the repeated history. Finalization preserves previous usage when adding the new thread.1782 regular tests passed; isolated PostgreSQL tests in both binaries cover fresh-recovery exhaustion, conflicts, retained accounting and additive fresh-thread finalization. A complete installed-runtime handshake fixture01a0a066-3b92-78a0-ba0b-f27c724fec2d used IDs1,5,2,6,3 and persisted exactly one message with no model turn. Image building in root session86022. Do not resume the old repeated context or general automation.
PR4392 merged 6a3a2f2a7325890de433c97606f8c523f74e2d5f; supervised worker image sha256:bb23e0a3e33b6bf74ab6915e2f86f9b9d2bf08a1458ad43edcf82376c8fc2a40 was verified running. Fresh continuation of the original task succeeded in thread 01a0a071-d0d7-7611-862c-e7d4a4b82669, with exactly one persisted validator message. Original implementation53ddb2b was retained; worker added bounded provenance in6496e8f315716015aac968b7b92f1a62e88d0d9c. Task accounting3530997 retains prior2915245 plus fresh615752. Original repeated history remains preserved.
Administrator Codex independently reviewed the complete three-file diff at exact6496e8f and reran18 focused tests across4files successfully. This is independent administrator review, not proof of a dedicated provider review-agent stage. The unchanged ChatModelSelector baseline failure remains the documented broad-suite exception.
The engine published and merged Azure PR6468 into dev: https://dev.azure.com/SkanAG/AI-Platform/_git/bc0e18b7-8626-49c5-8db3-d3ab2c8e0210/pullrequest/6468 . Merge revision efce189c674db8cfb8426febfaf04afd9cbd226e has successful development pipeline25712: https://dev.azure.com/SkanAG/4f0f0c9a-024b-43f0-9ee0-3de550130f03/_build/results?buildId=25712 . Exact scoped delivery continuation recorded outcome development_delivered and change-set state awaiting_operator_acceptance.
Verified HTTPS now serves entrypoint /assets/index-rpfiUVep.js and chat bundle /assets/ChatApp-C77L52lx.js, which contains the new parser. Executing the extracted served parser passed the expected title/description check. This does not replace authenticated end-to-end acceptance. The operator confirmed normal development-chat access. Review by sending a multiline /feedback command: inline title Feedback formatting check, followed by First description line. and Second description line. on separate lines. Expect title only once and both remaining lines in the description; cancel the draft after inspection to avoid creating another report. Original feedback41770 remains pending user acceptance. No TheWell production promotion or global automation restart occurred.
After the normal development-chat multiline /feedback test, the operator replied: "yes, that worked". This confirms the described title/description behavior and accepts this development fix at delivered revision efce189c674db8cfb8426febfaf04afd9cbd226e. It does not authorize production promotion or restarting general automation.
Completion-state limitation: current scoped delivery code returns early for awaiting_operator_acceptance and reports acceptance pending; no scoped acceptance-release command exists. This record captures actual user acceptance, but the engine change-set/feedback completion state has not been changed. Do not represent the remaining lifecycle transition as completed or clear it by globally restarting automation.
