The operator requested repair of stalled AutoCoder self-healing and actual issue/requirements progress. This is an external-administrator engine repair; it does not grant workers administrator permissions or waive product acceptance, subscription reserves, pacing or the existing supervised-pilot pause.
The original correction feedback43928 could not enter planning because its immutable admission binding referenced an old version of retained engine cycle97076. Later assessments deduplicated to the same pending correction; identical reconciliation changed the cycle version repeatedly.
PR11841 records append-only revalidation from a completed, fingerprint-bound assessment of the same feedback and retained cycle. Actual admission retains ownership/version fences, exact model and fresh lease checks, included-account capacity/reserves, stops and atomic rollback. Original work, correction metadata, proposals and history remain preserved. Identical lifecycle updates retain their version; material changes invalidate it. Controller revision is included in health observations without being treated as recovery proof.
Candidate4629 exposed an obsolete SQL-spelling assertion after account-load consolidation. PR11842 replaces it with a PostgreSQL behavior check proving guarded running admission, occupied-target rejection, queued redistribution, preserved identities/scopes and stable balanced work. REQ-0138 records the requested outcome-based testing and evidence-led recurring-fixture repair. Existing safety and protocol checks remain. Focused health checks:24 library/database and6 binary checks passed. Account-selection checks:3 passed with full migrations and canonical instruction fixtures. Candidate4634 passed all mandatory jobs.
Configured production workflow4638 succeeded. Deployment context: kubernetes-admin@kubernetes, cluster192.168.1.9, namespaceautocoder-engine. No role/credential expansion or ad-hoc runtime flag changes were made.

Retained cycle97076 version remained22:17:56.954561UTC across subsequent reads rather than changing on every identical pass.
Full worker publication4639 was requested through repository-managed publish-worker-image.yaml, using current main and exact deployed source38843b05. At this record, ordinary workers still select old imagesha256:78a75538b6787c410d150e316c0a553e3c2f3874a52abf77e026e5e838d6a4d3. Old worker startup migrations can redefine shared functions, so completion requires matching workers and another effective-function check. The configured publication script checks current-main tree and live-source coherence before/after build, immutable digest, existing deployment identity and its named migration Lease, and rolls worker-image selection back on failure. Active owners drain; only idle mismatched ordinary pods are replaced. Persistent tasks and the pilot pause remain preserved.
Original feedback43928 has not yet entered corrective planning; retained97076 remains delivery_blocked. No live recovery claim is made. The original goal—every remaining issue and requirement implemented or genuinely awaiting a human—remains unfinished.
Request4632 reported a read timeout although its immutable branch existed and exact candidate4634 was running; no duplicate submission was made. Preserve request evidence and repair ambiguous-result reconciliation at the shared dispatch contract. The started EventHorizon bell implementation stopped for missing prerequisite43966 evidence; it remains unfinished. Several human-wait labels describe AutoCoder-owned missing runtime evidence or recovery, without published human questions; they must not be counted as completed handoffs. These findings remain bounded follow-ups under the active engine-repair goal.
Task-owned temporary PostgreSQL fixtures and source copies were removed after validation. Borrowed validation container/shared compilation cache and original task history were preserved.
At2026-10-12 22:29UTC, the repository-managed publication succeeded and all seven ordinary worker pods were ready on worker01 with immutable image sha256:c4f49a0a1108637a8a476259ac72ea683ba10e46bea11b7637ddd3c7e31b6428. The worker source is main a00a1e87779337cf6bcc7e63c64727702acd2560, whose tree matches the validated deployed source38843b05. The new effective-binding SQL remained installed after rotation. Persistent work and the supervised-pilot pause were preserved. At22:39UTC the queued health assessment entered running with admission engine_revisiona00a1e87. Original correction43928 still had no corrective cycle at that observation; deployment and worker publication do not prove recovery.
The running updated-worker assessment engine-health-d22cfe956e62b30256bc0f0707b15c16cb8044f65405be471b2fc4423cb9f26a completed. At22:41:58UTC it recorded append-only revalidation event1 for original feedback43928 and retained cycle97076. At22:42:22UTC ordinary engine admission created corrective cycle138417 and atomic parking event2. The retained cycle remains delivery_blocked at the same22:17:56.954561 version with its original task identities, revision and history. Corrective planner self-improvement-plan-autocoder-autocoder-engine-1791844942264144721 is queued on an included account with exact confirmed modelgpt-6.1-sol; its next subscription outcome window is22:52:25UTC. This verifies recovery admission through AutoCoder; execution and delivery of the original retained work remain pending.
A further observation gap is confirmed:67 of186 open native issues have no linked cycle, and current health metadata omits older review/failed lifecycle evidence. A bounded visibility repair is under designated-worker validation; it has not been deployed. It must preserve private content, operator decisions and existing execution gates.
PR11920 passed21 focused admission/recovery checks and all candidate4656 jobs. Production4658 deployed sourceb2feebc2c2056faa8cfec7209394f049ac6bfdfd; healthy receipt2026-10-12 23:58:48UTC matches backendsha256:75cc6c2e66b64d9b4542af0ff593d554b251f251abbdec333e9f26516713f733 and frontendsha256:b36431ade59a8a447ffca098d8362342437e886e849cb674a41e98c884b36dfb. Corrective readiness SQL is installed. Matching worker publication4659 is running from maine0b81e81075768cdeae205442fc04ad5068bf234, same tree as the candidate. Live ordering after worker rotation remains pending. Read-only health tasks no longer own ordinary repository execution; fresh claimable governed corrections precede new diagnostics/retrospectives while same-session/resource/stop checks and admitted continuations remain. No extra admission or spending exemption is granted.
Corrective138417 advanced through planning and first implementation, then requested a plan revision. The revised implementation is blocked on a technical contract for trusted historical startup evidence; this is not a genuine human approval wait. The producer/trace contract must be established or unknown history retained with a safe existing recovery route. Planning lacked cycle scope and implementation consequently received a separate runtime outcome. Both spent records are preserved; future stage binding and appropriate historical accounting repair remain required. Original97076 stays parked and delivery_blocked, with its implementation, revision and history intact. No original-delivery or full-backlog completion claim is made.
PR11945 fixes the real planning producer's missing cycle identity before its runtime insertion hook. Planning keeps its read-only preparation mode; typed implementation, retries and verification inherit the same outcome/admission. Existing historical spent outcomes are unchanged. Seventeen designated-worker subscription/admission/lineage checks passed, including the real enqueue producer, cross-account continuation, retry, one job-start record and preservation of known runtime and an explicit accounting gap. The new regression is included by the mandatory candidate t_req_0100_subscription_ group.
Exact-head branch4661 and all candidate4663 jobs passed. Observed ready deployment images, backend health revision and served frontend revision match the candidate source. Matching worker publication4666 was dispatched through the existing workflow with source68297df3 and is running from exact main1d024cd3; live stage continuity remains pending.
Previous worker publication4659 is independently complete: all seven workers became ready on sha256:b410044a809055539ab55e9c26652eafedf924a8ea0e0a231eb75f680afe41a8, sourcee0b81e81 matching prior deployedb2fe. This new publication preserves active owners and replaces idle mismatched workers through existing digest/Lease/rollback checks. No account, permission, pilot pause, paid-use or reserve policy was changed.
Original recovery remains unfinished. Read-only inspection resolves the historical startup producer to the task's admission engine revisiona442fe6f4543aeb2dc36686383b889fc7f440d83: its account/read id5 error precedes thread requests. Historical progress/finish writers lacked claim-generation fencing; the final attempt is not proof of zero prior consumption or complete historical routing. Retain those unknowns. Cycle97076's completed implementation and separate failed baseline migration remain preserved; revised corrective138417 is blocked on the trust contract and must not be represented as a genuine human approval. The remaining issue/requirements goal is open.
The exact-main worker publication4666 succeeded. Live inspection confirmed six ordinary workers ready on immutable image sha256:b9b700c33b4f802f040efaabf74a8362a06e550d47cd5ab36a0b86edecfb25d2. Account32561's worker remains ready on its prior b410 image because its original EventHorizon implementation owns active execution; it was not interrupted for rotation. The paused pilot remains unchanged.
Production task self-improvement-repair-accepted-action-continuity-and-narrator-state-handoff-so-c138362 entered running at2026-10-13 00:38:21.13007UTC, on the original identity and worker. This proves execution resumed for preserved product work; implementation, review, delivery and original behavior acceptance remain unverified.
The latest completed health assessment inspected all18 native pages (188 open issues,67 unlinked) and identified distinct-report collapse, unchanged exhausted-gap version churn and additional diagnostic reader/ownership evidence gaps. Direct repair PR11975 preserves distinct unbound reports by semantic finding identity without granting another lane and makes identical exhausted-gap retention a no-op. Fifteen worker01 PostgreSQL checks and exact-head branchCI4669 passed at10e72410c189551a9f6b895bb51285cb320cddba. The new fixture initially failed the existing fingerprint guard; the fixture was corrected with the guard and explicit invalid-fingerprint rejection retained. Task-owned validator and disposable database were removed; the existing shared cache was mounted read-only and left unchanged. This repair is not yet verified live. Original startup recovery and all remaining issue/requirements outcomes remain open.
Candidate4671 passed all mandatory jobs, integrated into main0e4bf3144698ecd04c47c887c19134dd76c18c26 with the reviewed tree, and production4673 succeeded. Live acceptance contradicts complete version stability: corrective138417's exhausted implementation retained exactly the same error/state/command, but updated_at and reconciled_at moved from01:04:49 to01:08:38 and01:09:18UTC after deployment. The specialized gap writer is now a no-op; its caller also invokes the generic mark_task_reconciled writer, which unconditionally rewrites both timestamps. The previous local helper regression did not exercise this complete path. REQ0138T28 now retains the failed live evidence; T29 requires composed retention-plus-marking acceptance. A direct repair is under designated-worker validation: the shared marker updates only an absent marker or actual newer task evidence, with first-mark/new-evidence/idempotent-replay checks. No version-stability claim is made before live recheck.
Original startup recovery, historical accounting, EventHorizon implementation delivery and the complete native issue/requirements backlog remain open. No worker owner, pilot pause, cost gate, permission or issue history was overridden.
The full re-admission scanner reset reconciled_at for an already exhausted plan, reopening the unchanged task after the earlier local fixes. PR12060 changes the actual transition: exhausted blocked handoffs require correction instead of unchanged polling, and reconciliation bookkeeping no longer changes the material task version. Normal completed continuations remain eligible. Sixteen complete-cycle PostgreSQL checks passed on worker01; restoring the old scanner made the regression fail and restoring reviewed source passed again. T28/T29 retain earlier failed LIVE evidence rather than claiming those local checks proved recovery.
Candidate4685 failed because a minimal REQ0056 fixture omitted command_state. The authoritative downstream jobs were skipped; production stayed unchanged. The host monitor incorrectly described skipped jobs as success, retained as a separate bounded notification defect. PR12073 adds the production field and an exhausted implementation counterexample to the current-verifier fixture. Three library and one controller recovery checks passed; all native targets compiled, formatting passed and disposable task-owned validators/databases were removed. Runtime guards remained enforced.

Original corrective138417r1 was observed after deployment with updated_at03:17:46.903929UTC and reconciled_at03:19:45.468563UTC. A subsequent read preserved the complete snapshot, including error/state/command and both timestamps. This is an initial live complete-cycle stability observation; the underlying startup contract still blocks implementation. Historical work/accounting remain preserved.
EventHorizon original138362 completed implementation and obtained independent approval with reported811 tests and PostgreSQL lifecycle checks. Its subsequent delivery recovery is blocked on TEST storage-admission evidence around02:50–02:55UTC; no delivery acceptance is claimed. The administrator is tracing exact failed run3337 and configured admission/journal sources. The complete native issue/requirements goal remains open. Pilot21 and existing cost/permission controls remain unchanged.
All seven detached workers are now Ready on immutable image sha256:c9920554d9f97e2cab6828f45d5d959c472d92de12df82be21486bda9a16f031, published by4691 from main81fb. The scheduler no longer churns that unchanged gap.
Authoritative EventHorizon3337/job21148 logs show762 backend plus49 frontend tests passed, followed by timeout awaiting a reviewed TEST checkpoint at02:55:28UTC. Existing administrator service journals during02:50–02:55 repeatedly show NAS inspect checkpoint exit255. Read-only SSH diagnosis as actual service userax_julian established the FQDN lacked a known-host entry; the presented ed25519 fingerprintSHA256:AUaIFzBEo4QdWNRWkB7LPiYgdCIj6omDDz3jkUCyY6o matches the existing trusted kryptovisor short-host entry. A connection verified against that pre-existing alias succeeded. No host-key checking was bypassed and no new key was trusted.
Administrator repair atomically appended the FQDN for the three existing trusted keys to ax_julian's known_hosts, preserving owner/mode600 and all existing entries. BeforeSHA256:cef528a429e89bcad88044ca4b148d380636117b05b2b5b8b9fdda5addc52681; afterSHA256:bf9fb06688bdd9008a730f88a8294eaf98acaa6123e28857424c75dcc3817928. Restricted rollback copy is in the bounded operator task evidence directory; rollback must preserve any subsequent entries. Plain FQDN SSH now succeeds. The existing checkpoint controller reused the verified checkpoint at03:38:31 and published fresh schema-two admission (resourceVersion32676341, issued1791862709/expires1791863309). Both existing services subsequently reported success and continued refreshing. No snapshots, migration rollback, stored application data, worker rights, model provider access or production product workload were changed.
The retained delivery recovery remains blocked despite its pushed passing-CI checkpoint8b8c3c8fb913cff8e3bdbd9b0d2e1884158f48ab. Its existing native continuation accepts only completed recovery tasks, so it cannot currently use this repaired outside prerequisite. A scoped engine repair is under worker validation: authenticated, version-fenced continuation of the same blocked recovery with immutable private prior-task evidence and unchanged accounting/recovery limits. That source is not yet reviewed, integrated or deployed. The original terminal cleanup removed the task runtime; its existing one-time missing-rollout fallback is still unused(0), and accumulated164780effective tokens remain recorded. No new allowance or zero-use claim is authorized.
Goal remains incomplete: preserved engine startup recovery, EventHorizon TEST delivery and behavioral acceptance, and the remaining native issues/requirements are still open. Pilot21, main-account reserves and product production stops remain effective.
Source74e7ea985cdf65b51ca9111b0158a165f39a87a7 is published in PR12123 against dev (https://gitea.app.lan/AutoCoder/AutoCoder-Engine/pulls/12123). Branch CI4692 is running at that exact revision. It is not integrated or deployed, and no production native lifecycle row was manually changed.
Twenty-four REQ-0138 PostgreSQL regressions pass, including three new continuation tests with original evidence above1MiB, every-field accounting/work preservation, immutable private prior-task history, refusal for stale versions/evidence, dirty/changed checkpoints, parked cycles, exhausted continuation, open review/unanswered question, active child or competing integration, and replay without mutation. The new tests use the mandatory candidate filter, so they cannot silently remain unexecuted ignored checks. All native binaries compile; cargo formatting, explicit included-file formatting and diff checks pass. Earlier fixture failures were corrected against actual migrated ownership, parking, resource and uniqueness constraints; no production constraint was weakened. Task-owned validation and PostgreSQL containers on worker01 were removed by exact verified IDs; shared cache and other owners were untouched. Peak private target/cache/source was approximately7.3GiB with bounded resource/TTL settings.
Read-only current target proof:138362 is not parked, has no integration owner, no open review, no unanswered typed question and no active child. Its retained recovery branch still names exact8b8c3c8fb913cff8e3bdbd9b0d2e1884158f48ab; provider status is success from3339. The original task's approximately1.15MiB snapshot fits the new2MiB private audit bound. These are eligibility observations, not resumption or behavioral delivery proof. Next: exact-head branch CI, reviewed integration/full candidate, live runtime/schema validation, then authenticated continuation of that same task using freshly verified storage service evidence. Independent recovery review and TEST acceptance remain downstream.

The existing worker claimed it at04:39:06.495612 but failed repository preparation at04:39:06.685155: terminal cleanup had removed the exact checkout. The guard refused to recreate it from a different revision. Original output/reply/thread/accounting and missing-rollout allowance0 remain unchanged. Cycle returned to delivery_blocked; this is an observed failed continuation, not recovery success or a human decision.
The current producer is immutable worker81fb9cf52f0126c7d4b35ceeb96788f75eb51469. Its inspected Rust repository-preparation stage/captured-claim wrapper and worker/generation-fenced preparation finisher establish this current pre-task-command failure. They do not establish zero cumulative usage or resolve the older unfenced startup/accounting case.
Follow-up repair sourcea5788b9e69b435d6f5a1cc7f9af6ff9206f559c3 is published in PR12177 (https://gitea.app.lan/AutoCoder/AutoCoder-Engine/pulls/12177) against dev. Branch CI4697 is running at that exact revision. It restores only an unchanged published clean checkpoint, verifies actual HEAD/fingerprint/branch after cloning, protects unknown files/lost edits, records typed captured preparation failures, and permits one guarded continuation of the same pending administrator receipt before any new task command. The compatibility decoder accepts only the inspected81fb producer and unchanged original command evidence. Public cycle receipts append; private original evidence remains immutable. No recovery count, paid envelope, reserve, worker right or product production stop is reset.
Worker01 validation:26 mandatory REQ-0138 database checks and9 checkpoint/runtime checks pass, including3 new real-Git restoration/lost-edit/race tests and2 new preparation/producer-preservation regressions. New thread01a133e0-26d7-7b50-9356-3c485d36511a was recorded; the original thread and source failure stay in immutable history. Delivery recovery attempts remain1, verification-recovery attempts0.
The original recovery task completed06:15:33, pushed7efaaa154c1e9261ddc9b4c9bab9b0ffb2561dca and obtained successful exact-source CI3340 with reported811 tests plus PostgreSQL/format/artifact checks. The engine automatically queued a distinct independent verifier, which ran on account-confirmed gpt-6.1-sol and completed06:21:43. It then created and merged EventHorizon PR2386 into dev as12ed458a7ebad4a18d4ce90bdd10cf017105d402. Provider metadata independently confirms the merged PR and exact head. Native cycle is awaiting_delivery, and normal TEST pipeline3341 is running at exact12ed. No direct administrator product merge/deployment was substituted for engine recovery. TEST receipt and reported behavior, including the documented unavailable model-dependent acceptance, remain unverified.
Read-only investigation also confirmed original97076's initial implementation7cd711cf and forward startup/capacity repair commitsdcca274a/c4c05acd are ancestors of current engine main; latest d2 workflow-only tip is not. REQ0149 remains implementation_ready with unrun acceptance evidence, and its existing ignored database lifecycle test is absent from mandatory candidate filters. A bounded worker-local validation is running to establish that missing regression before any CI/source adjustment. This does not resolve97076 or the older ambiguous startup/accounting contract. Full native issue/requirements goal remains open.
Independent Gitea API and native provider rows confirm EventHorizon PR2386 merged into dev at 12ed458a7ebad4a18d4ce90bdd10cf017105d402, and TEST run3341/task19838/job21178 succeeded. Artifact5320 belongs to native run9893, exact source and candidate sha-12ed458a7ebad4a18d4ce90bdd10cf017105d402-run-9893-task-19838. Its unmodified schema2 receipt accepted_at is 2026-10-13T06:24:46.086882Z. It reports successful persisted persona edit, stale/invalid edit refusal, unchanged persona after failed edits and matching frontend assets. Historical unknown provider attempt remains null.
The existing event-horizon-test deployment UID20d5378f-13e0-48b4-9259-3b236418bd59 is Ready at observed generation11 with exact source12ed and image sha256:144b4932b6a9a70149e200b4c0221dcff65db4acf3ac8abfe133011170e7fa5e. Independent HTTPS healthz and readyz returned200 with PostgreSQL and Qdrant ready. No administrator product merge/deployment substituted for the engine's native continuation, independent verifier and delivery.
Native cycle138362 still reports delivery_blocked: its retained acceptance environment is descriptive prose referring to separately authorized model-capable production. TEST has deliberately disabled model access. Do not normalize that prose into TEST acceptance or claim gameplay verified. The engine has an authenticated managed-TEST observation importer; exact provider artifact/profile/review and current live evidence must bind through that supported route before a native delivery/acceptance claim. Production/model/capacity/pilot stops remain unchanged.
Testing review found two existing ignored PostgreSQL regressions omitted from candidate CI: source-independent human-decision delivery/resumption (REQ0149) and migrated-schema managed-TEST import CAS/replay. The first passed on the designated worker with one test and no failures. A source patch now includes both filters in existing isolated shards and strengthens wrong-owner answer preservation; validation and integration are pending. Existing mandatory checkpoint/preparation-recovery checks remain enabled. No chat subagents were used.
Engine PR12221 adds the previously omitted REQ0149 decision-delivery/resumption regression and migrated-schema managed-TEST import/replay regression to mandatory candidate database shards. It also verifies another authenticated owner's answer leaves all decision/cycle/feedback/review/case rows unchanged. Both database tests passed on worker01 isolated PostgreSQL16 (one each, zero failures), formatting/shell syntax/diff checks passed, and independent exact-source branch CI4703 succeeded for bf3c037d977e905212382f4bf66d1274b7877dd8. Parent source review74 was recorded as COMMENT without claiming an independent chat agent.
The unchanged reviewed source merged into dev59fc39b1615535845a92e14fcc610ff5d21aa1f5; both trees are5912dbe4558cd95f1fda7797e00e589e36d6c6d1. Release request4704/task19840 is running at that exact integrated source; full candidate execution remains pending. Exact owned isolated worker validator and PostgreSQL containers were removed after checks, preserving shared caches and other tasks. This patch changes tests and candidate coverage only. Original EventHorizon receipt import incompatibilities and model-dependent acceptance remain unresolved; the overall goal is unfinished.
Promotion4707 independently succeeded. Production is healthy at 2026-10-13T06:57:15.652945Z with source59fc39b1615535845a92e14fcc610ff5d21aa1f5/candidate4705-1 and backend a40b956ad9ef03454786c4dfa1a8f552e3e53bbcf023342e46ae6d2a56dd35a6, frontend3fa04e35d5a9f419fd9a67776bfef1fb63e6433b9cdc54f0f297e00a3334a5fa. Public health headers and frontend build revision independently match. Candidate job21181 logs show both previously omitted regressions actually passed, rather than relying on green workflow labels.
The existing managed TEST importer has two verified incompatibilities for original EventHorizon cycle138362: it requires a root-owned in-pod file despite backend UID10001, and it permits only ordinary verification while the native original recovery verifier is completed/approved at exact7efaaa154c1e9261ddc9b4c9bab9b0ffb2561dca with matching cycle/repository/requirements.
PR12241, source0a2b41133ef9234ce3842955d5967de058e629ca, replaces the obsolete file invocation with authentication before bounded exact-hash stdin for both inspection and execution. It permits the two established independent verifier kinds in profile validation and atomic import, preserving original review/checkpoint/profile/lifecycle/accounting/replay guards. The updated operator command is documented in the repository. No identities, permissions, DB schema, model/provider or managed production authority change.
On worker01, the observation group passed six checks including two actual migrated-schema database cases, and the stdin command passed three. Restoring only the original ordinary-verifier atomic import gate made the recovery regression fail while the ordinary case passed; restoring reviewed source passed both. Worker formatting, explicit included-file checks, diff checks and worker/controller hashes passed. Exact owned temporary validator/database containers were removed after checks, preserving shared caches. Parent review75 was recorded as COMMENT; no independent chat agent review is claimed. Branch CI4708/task19847 is running at exact0a2b. This repair is not yet merged or deployed, and no live evidence import or gameplay acceptance is claimed.
Independent native provider evidence binds EventHorizon artifact5320/run9893/task19838/job21178 with native attempt1/statuses successful and original receipt2405 bytes, SHA256122b80eb2740bd6f45c612538729bd33d0e75bc0e99f552a5dd52ccc20f36a63. Null historical receipt attempt remains null. Current live TEST source/image/config match that artifact, and actual git inclusion checks passed for both reviewed7ef and integrated12ed into deployed12ed. Private source observations remain restricted to the task evidence folder; revalidate freshness and original ownership before the supported live import. Original model-dependent acceptance stays pending under existing TEST/provider and production stops.
Engine production promotion4712 independently succeeded: source891b85dfc22f75fabbaadbf1dea0a74971d6a082/candidate4710-1 is healthy at07:30:49.804791Z. Backend f03f4e5744f22fab666821b5a76e2fbbb8dfb9039a31a7d24a3f5cfd290368a5, frontend0c9474293899f58fac59aba64c52fa55c469f543aa293aff99d1dc80a34986a1, health headers/frontend revision independently match. Candidate job21189 proves both ordinary/recovery import DB tests and all three stdin tests passed.
The repaired authenticated stdin command successfully inspected and imported the fresh native ManagedTestObservation for original cycle138362. Envelope52286 bytes/SHA4dcfed460865378e0d098a8ce4f453e6f762fdf0f40583fbb6aab2b5d8678e8b observed07:33:30.154575Z; provider PR2386 merge time06:22:00Z and original receipt deployment time06:24:46.086882Z stay distinct. Retained binding fingerprintaeee69684b16c9e67cc13a30af4b28e82e167a0ee87222e3780958cf862c3512. An actual replay before acceptance completed changed neither original task identity/account/model/checkpoints/token accounting, recovery allowances1/0, original requirements, nor receipt/binding; exactly one acceptance task existed.
AutoCoder then queued and ran its original-cycle live acceptance task on the confirmed gpt-6.1-sol worker, thread01a1342f-7ad0-7763-b46b-6a12cc79945a. It observed healthy ordinary TEST endpoints and the retained synthetic persona, but reported unresolved runtime-image read capability and model/browser capability. No gameplay acceptance passed, and no worker permissions, provider access, paid allowance or product production authority were expanded.
The task exposed another recurring contract failure: it returned checks[].evidence as structured objects, while the engine DTO required text, yielding invalid type: map, expected a string. The engine converted that technical failure into review_required; native issue2916 became waiting_for_human although the cycle has no review request, escalation or outbox record. This is not a genuine human handoff. A subsequent engine repair is in progress to preserve structured/text evidence, keep completed technical gaps engine-owned, preserve real questions/stops/accounting, and retain original review identity after appended acceptance failures. The existing source-string test incorrectly insisted on review_required; it is being replaced with behavioral state/ownership coverage. Source edits and validation are pending; no further fix or full goal completion is claimed.
PR12311, source00d4fd585e8eff723ba4d5b825132974bc05cf98, preserves structured evidence as canonical JSON text, recovers typed capability findings from the original completed result, and retains technical gaps in the existing failed/blocked lifecycle under AutoCoder ownership. It releases the integration slot without changing tasks, accounting, original review, retry limits or permissions. Actual open reviews, operator gates, withdrawn cases and active work prevent conversion. The engine-only parked-cycle contract stays unchanged; no new state or DB schema is introduced.
Original-review comparisons in profile validation, acceptance context and both final-outcome writers exclude only controller-appended acceptance_failure metadata. The failed observation remains recorded while its original approval and TEST binding remain usable. No failed check becomes passing and no task/human question is fabricated.
Worker01 isolated validation passed the technical-gap database regression, six managed TEST observation checks and nine native acceptance checks. The lifecycle regression tests real case/card projection, unchanged complete task rows, true review/stop preservation, stable replay, no duplicate observer/question and admission of another outcome. Restoring only the old decoder and omitting the lifecycle hook reproduced both original failures; restoring reviewed code passed both new regressions again. All seven tested source hashes matched, formatting and diff checks passed, and exact owned temporary validator/database containers were removed after checks.
Parent source review76 is COMMENT, without claiming an independent chat agent. Exact-head branch CI4713/task19858 is running. The current live original cycle138362/card2916 has not yet received this correction. Its model-dependent gameplay acceptance remains unverified, and the overall issues/requirements goal is unfinished. No worker permissions, provider/paid allowance, pilot pause, managed product production authority or user history were changed.
PR12311 exact-source branch CI4713 succeeded and reviewed source00d4fd585e8eff723ba4d5b825132974bc05cf98 merged into devc00695ede56432e31ac681feb0eb3ceede3cae30, treee310be3d5057034082a5e8ef4881942b2184a6f4. Full candidate4715 failed in two older minimal PostgreSQL fixtures, t_req_0094_database_resolution_and_exhausted_recovery_are_truthful and t_req_0136_t_req_0150_database_feedback_authority_is_monotonic, because retained acceptance reconciliation now requires codex_worker_tasks and review-gate relations. The newly added parser/state regressions and managed TEST recovery case were observed passing in the candidate log.
Native Gitea task19860/job21200 failed; downstream tasks19861–19863 are skipped (native status4), contradicting monitor success labels. Live production and TEST receipts remain source891b/candidate4710-1. The repair is integrated into development but not deployed, and original case138362/card2916 has not received the correction.
A test-only correction now adds the current observer/review/escalation relations and cycle fields to those two older temporary fixtures, retaining all authority, history and truthful-resolution assertions. No production missing-table bypass is added. Isolated worker01 validation is running; no unchanged candidate retry or additional runtime, model, permission, provider, spending or managed production change is claimed.
PR12332, source4c4f9eb13bab8cb5104c361f974707b7d5abd793, updates only the two older temporary fixtures that caused candidate4715 to fail. It adds current retained observer/review relations and cycle fields, reuses the temporary worker table in later recovery setup and makes repeated column additions idempotent. Original authority/history/resolution assertions remain intact; no runtime guard or production schema change is introduced.
Both affected regressions passed on worker01 isolated PostgreSQL16 with canonical fixtures. Formatting, source hash and diff checks passed. Parent source review77 is COMMENT, with no independent chat agent review claimed. Exact owned temporary validator/database containers were removed after validation. Branch CI4716/task19864 is running; development integration, full candidate execution and actual live correction of original138362/2916 remain pending. Production and TEST continue on the verified source891b/candidate4710-1.
Candidate 4718 failed natively at task19866/job21206 because the optional acceptance fixture lacked observation.result_reply. Downstream tasks were skipped; there was no new deployment. Production remains source891b85df/candidate4710-1.
PR12345 source940f8ea535639f4b13501cf6419f6544f41fcc6f adds current fixture fields and the genuine escalation relation. Audit also found the existing native intake concurrency/replay regression was omitted from candidate filters and lacked the production lane functions. Its fixture now installs the actual health/parking SQL; candidate shard0 must execute it. Both native database tests executed and passed on worker01 against disposable PostgreSQL16. Formatting, transferred source hashes and diff checks passed; exact owned test containers were removed without touching shared cache. Parent COMMENT review78 recorded; branch validation4719/task19872 running, full candidate/deployment pending.
Current native inventory verified:189 open,168 blocked,10 waiting for AutoCoder,9 labelled waiting for human,2 working. Latest additional item2968/cycle139928 has a running EventHorizon implementation. Original2916/cycle138362 reached TEST through AutoCoder PR2386 but gameplay acceptance is unverified and its technical failure is incorrectly labelled human wait in current891b. The correction awaits full candidate validation. No verified issue closure has been delivered by this repair. The human explicitly measures progress as completed original issues and requirements, not patch activity. Next acceptance checkpoint must trace preserved blocked work through actual recovery and final outcome; avoid new diagnostics outrunning it. All original histories, accounting, protected capacity, pilot and production stops remain effective; no chat subagents or Gitea issue mutations authorized.
All four candidate4721 jobs independently succeeded at source8190cc7e117c02fdee4709a1dad25a236fbcee9e. Job21214 logs prove native intake, optional acceptance, technical-gap ownership and both managed TEST replay database checks actually executed and passed. Main5306a5d2159f2570e695aae6563ade009763484e has the same tree22f6c10971548b07a61dde03c2d0913ab2780dd4. Promotion4723 succeeded. Public health returns200/JSONok/source8190cc7e/production; frontend package revision matches.
The actual startup reconciliation reparsed retained original acceptance evidence for138362/2916: cycle nowfailed, technical_acceptance_gap.owner=AutoCoder, failure kindcapability_unavailable, previous classificationevidence_unavailable retained. Native2916 is blocked/verification rather than falsely waiting for a human. No original issue closure or gameplay acceptance is inferred.
Next selected work is original engine1491/cycle97076. Its retained baseline migration failed on disabled API-compatible accountopenai-compatible-dacb49487080a186ee492190. The complete673-byte trace contains initialize, disabled remote-control status and account/read -32600 before a thread. Full attributed sourcea442fe6f4543aeb2dc36686383b889fc7f440d83 confirms that order and immediate error propagation. Old worker pod is absent and lease expired. Clean checkpointbcc876d9bd9aa3d7bc187c0eb7a782f77b3c1da4 exists and its recomputed current fingerprint112597e5dd023c5d2f14578a91de02308ba0290f6d0eaf488dde66112d74d645 matches exactly; original6 provider checks are green. Its migration branch was never published. Correction138417 remains intrinsically exhausted: authoritative r1 is blocked/plan_revision_exhausted while cycle incorrectly retains implementation ownership. A bounded administrator migration is implemented in a separate branch, preserving the same task, unknown historical usage, old counters and private archive; tests cover actual stops and resource/account/model guards. It remains undeployed and original1491 remains blocked. Native cancel_reason contains the technical diagnosis here, so cancellation must be determined from actual typed state/status, not that field alone.
PR12387/candidate4726 sourcea1013cd9 and refinementPR12417/candidate4732 sourcebc821914 were independently validated and promoted. Mainff2ea2ae has the same reviewed treeccd9e8fc; promotion4734 succeeded. The original unpublished initial branch was restored atBCC through normal authenticated Git after a native branch-create API422. That old snapshot queued run4729 with retiredrust-large labels; no active runner supports it. The branch later disappeared before continuation. The same recovery branch was safely created at current canonicalbc821914, containing the old checkpoint as an ancestor; current branch validation4735 succeeded. No force/history rewrite, retired runner or old deployment was enabled.
The authenticated engine command preview succeeded, then execute succeeded once at12:47:54.729662 on SAME original task self-improvement-necessary-human-decisions-can-enter-a-durable-review-state-but-l-c97076-baseline_migration-recovery/cycle97076/native1491. It uses eligible non-paid subscription accounta8552b49.../modelgpt-6.1-sol, original queue identity and scope. The immutable old task archive exactly matches the private pre-migration snapshot; historical usageNULL remainsNULL, oldcheckpointBCC retained, initialcheckoutbc821914, recovery counters2/1 unchanged. Cycle97076 is delivery_recovery_queued/unparked; intrinsically exhausted corrective138417 is failed with old blocked task unchanged. No original issue closure is inferred.
The task nevertheless staysqueued: first-start governor treats its old outcome as new. Historical tasks lack selected admission receipts/cost segments, so existing strict adoption cannot safely reconstruct subscription start. At12:58:41 a new health assessment took the window, and newdiagnostics repeatedly defer the actual restored delivery while retainingpriority90 versusdiagnostic80. Preserve the fifteen-minute policy and unknown accounting; repair ordering instead of manufacturing historical admission. PR12459 sourcecfd0ca7b extends protected ready-correction ordering only through the existing authenticated private receipt and exact producer/task/cycle binding. Public markers/wrongactors do not grant priority; current claim-time scope/account/model/lease/capacity/stops still apply. Allnine scheduler checks passed on worker01; old predicate reproduced a NEW investigation taking the recovery's next admission, fix reproduced the actual original delivery claim. Owned fixtures were removed, shared cache untouched. ParentCOMMENTreview81 and branch4737success; reviewed mergeDEV3b8e3e9f treead8b4d88; fullcandidate4739 running. No actual SDK claim, implementation or issue closure yet. The next acceptance checkpoint is this original task's claim and delivery, not morediagnostics.
Native issue 1491 / retained cycle 97076 was actually claimed at 13:28:42 UTC and failed at 13:28:43 during repository preparation, before any provider thread. The exact recovery branch returned 404. Gitea's read-only action history records StockFish deleting that same branch at 12:03:18 and 12:58:03 after administrator restoration; repository lifecycle reconciliation treated a clean checkout already contained in dev as deletable without checking unfinished work ownership.
Repair PR 12493, source f958a95e1dbd9f4befe9799a96d4e54a58e52f46, preserves unfinished branches and rechecks ownership before provider mutations. A once-only authenticated preparation retry preserves the same original task, scope, account/model, clean checkpoint, unknown usage, counters, runtime and first successful admission. The original startup migration receipt remains immutable; the separate unused preparation receipt archives the exact failed claim. Seven relevant regression tests passed on worker01, including actual provider-action refusal; the strengthened preparation test passed again. Server build and required formatting passed. Branch validation run 4742 is pending; this repair is not deployed and issue 1491 is unresolved.
Live inventory: 189 open native issues (170 blocked, 10 waiting for AutoCoder, 8 marked waiting for a human, 1 working). No verified issue closures from this repair. Issue 2916 is blocked with an empty category and workflow stage verification. Its PR 2386 and 06:24 UTC TEST deployment receipt exist, but original gameplay acceptance is unverified; this must not be reported as issue completion or current TEST-stage status.
Branch validation 4742 was independently checked against the provider API and native read-only Gitea task/job records: exact source f958a95e1dbd9f4befe9799a96d4e54a58e52f46, task 19917/job 21263, both native status 1 (success). PR12493 was merged into dev as 19e9e60a1c27de158d10a2e4bb1fbfb65385e187; its tree f52446b47819ef387fb36db56e4de7d31eb213e0 exactly matches the reviewed source. Release request 4743 succeeded and full candidate 4744 is running for that exact development revision. Production deployment and original issue1491 recovery remain pending; zero verified issue closures.
Full candidate4744 was independently checked in native read-only Gitea records: candidate task19919/job21265, finalize19920/21266, integrate19921/21267 and deploy19922/21268 all have native task and job status1. Candidate logs show the new branch-retention, preparation-history/replay and actual provider-deletion guard database tests passing in the mandatory isolated DB stage. Automatic main PR12502 merged as dda19534ab9bb8990b8426ce45f0f60db423d390; its tree f52446b47819ef387fb36db56e4de7d31eb213e0 exactly equals reviewed dev19e9e60a. Production request4745 succeeded and promotion4746 is running. The public engine still reports source3b8e3e9f, so branch restoration and authenticated same-task preparation retry remain dependent on verified live rollout. Issue1491 remains unresolved; no issue closure claimed.
Promotion4746 task19924/job21270 has native task/job status1. The public engine health and frontend build both report source19e9e60a1c27de158d10a2e4bb1fbfb65385e187. The exact original recovery branch was restored at retained checkpointbc82191498bd3b5c179f31a142e382565239af2b. Authenticated preparation-retry preview and execution both succeeded for the same cycle97076/task; no direct lifecycle SQL writes or manual issue state changes were used. The private once-only receipt preserves the actual failed claim, scope and usage; its only difference from the earlier administrator read is the scheduler's reconciled_at bookkeeping. The original startup archive and first successful outcome admission remain unchanged.
The original worker then passed repository preparation, started thread01a135b4-d8c5-7a21-bcb9-77bdaaad91cd, and completed successfully. It published09c3f9e8008a491ed877f925c6d6405cbace137e: a reviewable baseline-preservation record because the original approved implementation and remaining CI intent already exist on validated dev. Branch validation4748 task19926/job21272 is natively successful. The engine has scheduled and started a fresh independent review task ending baseline_migration-recovery-verify on included capacity. Native issue1491 is now working, typebug, delivery stage. This proves original-task execution and advancement into review, not the original behavior's live acceptance or issue closure. Zero issue closures are claimed.
The original baseline-migration verification task completed successfully with an approve verdict for exact09c3f9e8008a491ed877f925c6d6405cbace137e. It independently mapped the preserved implementation and two-line CI intent into validated development and explicitly retained original-case live behavior/acceptance as downstream work. The engine itself created and merged PR12515 into dev8396a199076c88c256613c9d0e6ee13f7027292c. Original cycle97076 now owns integration and awaits delivery; native1491 currently waits for AutoCoder while candidate4750 runs for that exact source. This is original-task execution, independent review and governed development integration through the repaired engine. It is not original behavior acceptance or issue closure. The current application-equivalence gap must be resolved by normal delivery reconciliation, not reclassified as a human wait.
Candidate4750 and production4752 were independently checked in native Gitea records: all candidate jobs and promotion task19933/job21279 succeeded. Main PR12524 integrated exact dev8396a199076c88c256613c9d0e6ee13f7027292c as ac0f606fe09b846a9f9f125bd341a889c4b6acc8. Both production and TEST serve source8396; the canonical ledger retains candidate8396a199076c88c256613c9d0e6ee13f7027292c-4750-1, backendsha256:185f7266f08c1cb0015a3f361228d926ae6390e6475e2071bcd11e6f539a3100 and frontendsha256:f7867ff95e84a27632cba7457430007b31e53789c6cc50563207ce7f5580e1c6. TEST's verified ingress is https://test.codecartography.dev.
The original feedback consumer waited for autocoder/deployment/production, which the canonical publisher omitted. After fresh production backend/frontend source checks, an administrator projected the acknowledged canonical receipt onto exact source8396 in Gitea status11, linking promotion4752. No issue state was manually changed. This allowed AutoCoder to start the original live-acceptance task. The observer verified deployment but failed the behavior checks: operator authentication and bounded original-case audit/intake/answer/retry evidence were unavailable. The original cycle is now failed and native1491 blocked with AutoCoder owning that technical acceptance gap; no closure or human wait is claimed.
Existing administrator Gitea identity ax_bechju1 was verified through the normal production /api/auth/session bearer-PAT path; operator API access succeeds. That credential is not shared with workers. Its escalation view contains one item; no open AutoCoder-Engine escalation or review request currently exists. The legacy info@codecartography.dev identity does not equal that operator's Gitea email; no identity migration or permission change is justified by these observations alone. The worker admission's engine_revision bdf985... identifies its worker build, distinct from deployed backend8396; source attribution requires component-aware evidence, not rewriting historical claims.
Persistent publisher repair PR12565 passed branch4753 and all24 transport/identity tests plus the full candidate-admission suite onworker01. Its exact c177f5635d9532ac9a22f5d2e443145f6e2328cd was reviewed and mergeddev9e4caefb9b1894ed19ad4ddf72f02310c245f350 with matching tree87104cadc8df77c7f3c199073a7c63f5c22d0e60. Candidate4755 is running. It publishes the verified compact receipt after canonical ACK; bounded publication failure exits75 and retains the verified healthy rollout. Original behavior acceptance still requires authorized normal-runtime evidence; another unchanged observer retry is not a repair.
Native task19936/job21282 failed in the server unit suite; finalize/integrate/deploy jobs were skipped (native status4), despite the monitor rendering them as successful. The publisher tests were successful. Existing test stale_tmp_alias_for_another_task_fails_closed_and_preserves_its_target failed before its alias assertion while immediately reacquiring a dropped fixture flock. The same module already documents parallel-fork inheritance and contains a two-second, WouldBlock-only reacquisition helper. PR12580, exact source9512d2700d89b12191a73de4b886dca1b898f75c, reuses that helper in the missed fixture. Full server suite passed onworker01:1096 passed,55 intentionally ignored; formatting/diff checks passed. This is test-only, preserving the real ownership refusal and stale-alias assertions. Branch4757 is running; original issue1491 remains blocked on unverified behavior, not this test fixture alone.
Native AutoCoder issue #1491 remains unresolved. PR12600 restores the authenticated Overview decision surface and owner-scoped first-party delivery. PR12629 retains the original feedback/cycle on approval, atomically queues one planning continuation on eligible capacity, preserves the runtime outcome and first subscription admission, and recovers still-owned answers after interruption. Rejection, scoped stops and existing capacity rules remain effective.
Source 08713d89c93d3eb0e750d72a6844c3f12d6c255e was reviewed and integrated into dev504fdff8dec57fa7573958f84bee7bc6cecc8193; main14745fea48f67295035796458a76c3caf2c6ed1e has the same tree ef543f6947274d430a47188746a1cf8ea6535f0d. All four native candidate4767 jobs and native production4769 task19962/job21308 succeeded. Both production and TEST backend/ frontend serve source504fdff8. The pipeline automatically published the genuine production receipt: backend sha256:36830f09f2baa48b59d9ed0abfff2ad389c96bc4989a791744525e9633fa74b8, observed2026-10-13T18:21:21.351260+00:00.
Worker01 validation: three REQ0149 database regressions,555 library unit tests,1096 server unit tests and the33-test recovery group passed. The previous candidate4764 was cut off by the fixed120-second recovery-group deadline; validation now inventories selected tests and applies a bounded120–600-second deadline without omitting coverage. Owned test containers and task-scoped registry authentication material were removed. No chat subagents or manual issue mutations were used.
These checks do not prove deployed task continuation. TEST uses an isolated database and authenticated first-party UI but disables background automation and implementation workers. Production has those flags enabled. A live queue inspection at18:41UTC found three queued tasks and none running: a health assessment, the preserved paused pilot acceptance, and an eligible engine-correction planner. The next shared15-minute admission is18:44:01UTC. Recent admissions were health/wrap-up reviews; the corrective-before-new-review rule already exists in the running worker source, so observing the next actual selection is required before attributing starvation to another source defect. Pilot generation21 remains paused. No budget, cadence, permission or history resets were applied. Verified issue closures from this repair remain zero.
PR12711 preserves structured interfaces_and_data descriptions without changing strict scope fields and adds authenticated administrator recovery of a retained successful planner result rejected during JSON finalization. Worker01 validation passed 12 plan tests, the isolated recovery database regression and 1,096 server tests. Failed original output, usage, queue owner and runtime remain preserved; paused or superseded selected work is rejected.
All four native candidate4774 jobs succeeded. Source53025418bb26ed26b41aab5a159e8a3fb8db657d is deployed to production and TEST, backendsha256:4e8e4eca6f59454de7534f21e6d32a6b8adc5ab078b5cf4879c23915215766ec, frontendsha256:42c4a4dc68d3d3fdbc52d751022fc7b5486f6f7c7c9c2ea4c3ccd279f52809e7, deploymentannotation2026-10-13T20:32:58Z. Cluster/context and immutable source/digests were checked; no ad-hoc flags, permission expansions or pilot changes occurred.
The engine service-account-authenticated recovery successfully advanced original TheWell change set sp-requirements-1791614737802715064 from planning into implementation, using retained artifactSHA256 ebfffee2f2550a177f997ecc488c2b4f5579a05c3ac9d583d94dc41039e1d11b. This is a deterministic reconstruction of its recorded writer and immutable original inputs, not an independently recorded original file hash. The original planner stays failed with its original timestamp/output. The successor preserves the same requirementREQ0001, actual queue owner and scope.
Actual recovery then failed validation at20:34:39UTC without starting an SDK thread or publishing any repository. The existing-implementation shortcut trusted historical requirement metadata and skipped the model despite this plan requiring additional catalog repair. Its source validator failed on the stale95-record baseline (current96); .NET restore/test failed because /.dotnet was unwritable. A clean current source checkpointc1e14e5eeded88e08de47ba98834f32e27387985 and exact failure results are retained. These are AutoCoder-owned technical gaps, not human handoffs. Repair now targets current executable evidence before skipping work, task-owned validation home, and a once-only authenticated continuation of this unpublished no-SDK failure. Changed baseline must be revised while preserving intervening rows; no unchanged validation replay is authorized by this record.
New retained-plan DB test passed manually but was omitted by fixed candidate filters; its registration is being corrected. Native engine3034 remains running with two actual commits; delivery is still unverified. Original1491/2916 and all remaining backlog outcomes remain unfinished. Verified issue closures from this repair remain zero.
All four native candidate4786 jobs succeeded. PR12747 is integrated as source3fff70ae035c80dfb8123774350bee89564a7249, deployed at2026-10-13T21:21:27Z with backendsha256:91bd3634d7b50be7b45b2d46f8a14049e62f460dd1971b0ffed84d82d1e96277. The runtime requires current executable plan checks and an unchanged checkout before skipping SDK work; .NET validation has a private writable Git metadata home. Worker01 source checks passed1,097 server tests and the guarded replay/SDK continuation database regression. New recovery tests are registered in mandatory candidate groups.
The normal requirements reconciler superseded older TheWell implementation1791614737802715064 with current successor1791924869510690670 for the same REQ0001. That successor also failed validation without an SDK thread or provider consumption, retaining the clean current checkpointc1e14e5eeded88e08de47ba98834f32e27387985 and exact validation failure. The older superseded task must not be restored. A narrow followup removes the predecessor-only administrator artifact marker from SDK continuation eligibility while retaining authenticated engine-SA execution, exact current failed ownership, unpublished clean checkpoint, no SDK result/usage, current member write authority, active-owner/pause guards and once-only private failure archival. The successor-without-marker database regression passed, including SDK/thread/result/provider/total-usage counterexamples. Followup deployment is still pending.
Native issue3034 did complete implementation, independent gpt-6.1-sol review and normal dev merge through PR12746. All four candidate4781 jobs succeeded. Its real live acceptance nevertheless failed: supported task-status/capacity HTTP calls returned401 to the verifier, and available scoped observations did not expose original diagnostic bindings/history or any original diagnostic recovery. The original engine-diagnostic-f9b8ef469095cf2f-1791804774437516843 remains failed on its recorded singleton-scope rejection. The worker image still needs the repaired scope readers; publication of the exact current production source is being dispatched through the repository-managed workflow, preserving active owners. After publication, recover the preserved original through a supported guarded route and establish authenticated, bounded runtime evidence without lending operator credentials or inventing a human wait. Source fixtures and deployment alone do not close3034,1491 or the original case. Verified issue closures remain zero.
Native worker publication4790 task19992/job21338 succeeded from exact main d75d07324c56cc9618ff38cf18c79bd3a518cc35 (same tree as deployed3fff). At21:43UTC all seven ordinary worker pods were ready on immutable sha256:5892e9af4ac73ef52e3189bdd594b032101c49248fce0e6b35cc2dae57f14f77 on worker01. The active3b0f owner drained normally; it was not interrupted. The paused generation21 pilot pod and its older image were untouched. The backend selects the published worker digest through the repository-managed publication. No new roles, paid capacity or arbitrary runtime flags were introduced.
Already reviewed PR12771 and native branch4789 passed; its current-owner continuation guard is integrated into dev8624c67c8cffd20ec734d0c9f1f1728b85e52a0a, same reviewed tree7bac53fd7a948a863c0ca589a85622a2a974b082. Candidate4792 is running for that exact source. Production still runs3fff, so the newer SDK continuation guard is not yet live. After publication, resume only current failed TheWell task superproject-requirements-1791924869510690670-implementation through its authenticated guarded command, and verify actual SDK execution rather than another metadata shortcut. Older superseded work stays preserved.
Original diagnostic f9 remains failed. The single-task worker command only executes queued work and does not requeue a failed task; no supported diagnostic retry command was found in the inspected operator/controller paths. A failed-task recovery must use an existing verified store route or a narrowly fenced administrator transition preserving scope/history/accounting; direct production SQL and a blind new task are not authorized substitutes. No original-task recovery or issue closure is claimed. Original1491 and native3034 live acceptance remain failed on unavailable authenticated/attributable runtime evidence.
All four native candidate4792 jobs succeeded, and source8624c67c8cffd20ec734d0c9f1f1728b85e52a0a is deployed with backendsha256:d93d40855a59143175017c19a968ca2b32e42ce742480417f3655169f0ce57d0. The current1791924869 task was already superseded by normal recovery; the guarded preview rejected it without mutation. The authoritative successor superproject-requirements-1791927476595613334-implementation is actually running on published worker5892/accounta8552 with gpt-6.1-sol and SDK thread01a13741-d150-7161-a1c5-6468269222e7. It changed the canonical requirements catalog; this is execution, not completed validation or delivery.
Investigation found dotnet restore PID338, shell335, engine parent7, still alive after more than25 minutes despite the configured600-second validation timeout. Its exact cwd belonged to this task's backend test project; its parent command exactly matched the captured validator. The asynchronous output future had timed out without terminating the process. The SDK's reported compiler activity did not establish what this separate process was doing. Administrator sent SIGTERM only to this verified orphan338, confirmed it stopped, and preserved the SDK process and all files/history. No broad PID matching, lock removal or session interruption occurred; no claim is made that the orphan caused all build delay.
PR12815 runs validation in its own process group and uses the existing owned-process guard to terminate children on timeout/cancellation, preserving successful output. Twelve worker01 validation tests passed, one DB test intentionally ignored. The new regression proves timed-out parent/child stop while unrelated work remains alive. Source0c29cb497a088f08bd5f578446856259bae1ba74 is reviewed, branch4795 running; this cleanup is not yet deployed. Current SDK work and original live acceptance remain unfinished. Zero verified issue closures.
The current TheWell SDK completed successfully, reporting one passed catalog test, zero failures and all96 records preserved. Commit a2b27ed3298b3c07af157cfc00daf82626ab4113 and clean checkpoint fingerprint3d9cf84092af067bf9d9545c4b3efe4c75d43b239885287834947a871a5d4284 are retained. This is source/test evidence, not development delivery.
The independent finalizer ran dotnet restore again and timed out after600seconds. At22:24:13UTC the task becamefailed with SDK exit0 and matching result thread preserved. The change set isfailed with outcomes[] because timeout interrupted outcome persistence. No repository was published. Its validation home contained239MiB of packages; differences from the SDK cache/feed environment need investigation, not assumption. Preserve the committed source and use guarded validation-only recovery after repair; do not restart implementation or invent a human gate.
PR12815 timeout cleanup passed native branch4795 and merged into dev0aadbeb41ce6fe5fa9a21c1327520fc10ba58229 with reviewed treec8f92d8205f115ae8007bb4b9313c671bb2a1bb1. Candidate4797 is running. Cleanup requires deployment and matching workers; it does not itself repair package retrieval or recover publication. Zero verified issue closures.
All four native candidate4797 jobs succeeded and source0aadbeb41ce6fe5fa9a21c1327520fc10ba58229 is deployed, backendsha256:206e2b4db57ebb6fe7d72b5a43fa88b85e026ca29f2429f028ad71ccf9bc3d9f. Worker publication for this runtime change remains necessary.
The SDK assets file records151 libraries under task runtime scratch/nuget. That directory was removed by normal terminal scratch cleanup. The independent finalizer instead used Git metadata HOME and its own package directory, causing a second restore rather than reusing SDK dependencies. The later verified orphan restore4272 (shell4269/engine7, exact task project cwd/command, elapsed20minutes) was stopped with targeted SIGTERM; source and result history remain intact.
PR12838 makes both SDK execution routes and finalizer share private task-owned Git metadata NuGet/Dotnet paths. Timeout remains a failed structured validation result, preserving publication refusal and recoverable outcomes. Guarded validation-only replay accepts the earlier exact SDK-success timeout with empty outcomes only with matching provider/result thread, clean checkpoint and unchanged current ownership/authority. Worker01 database regression and13 validation checks passed, including shared package reuse, wrong-result/generic-failure rejection and timeout process cleanup. Source c5d0d112dc6f79a8eb63acacacbc271448896cc6 is reviewed; branch4800 is running. No actual delivery is claimed.
Normal supersession replaced the failed successful-SDK task again at22:31:38 with implementation1791930398467051114, which is now running SDK thread01a1376d-af7c-76e0-973b-94ec566e7a9b. The earlier a2b27ed commit/result remain saved but its error/state are superseded. Preventing repeat planning from outrunning delivery recovery remains an engine-owned gap; do not restore a superseded owner while its successor runs, fabricate a passed check, or treat this as human intervention. Zero verified issue closures.

Candidate4802 is running; production and workers do not yet have this package-environment repair.
The authoritative current TheWell implementation1791930398467051114 remains running with an actual SDK thread and a catalog worktree change. Preserve this owner. Earlier successful SDK commit a2b27ed and its result remain retained despite normal supersession; it is not development delivery. Read-only PID/cwd/command/parent checks proved precheck restore3189/shell3186/engine7 belonged to this same task and had exceeded its deadline for30minutes. Worker publication run 4805 at main df2e905c6daa01b2c761144d863fdc06aad5caf8 (same source tree as backend source 04abd556eb69cd97c1302b9440f1073cdf3bdf35) passed native task 20016/job 21362; task and job statuses both 1. All seven ordinary workers were observed Ready on worker-01 with immutable image sha256:3a1458a39a8d84e4a9888d1983e3eaa84252800ff8399f20839c96aa26e7d98f. Paused pilot and its old image were left untouched.
The original current TheWell task superproject-requirements-1791930398467051114-implementation failed its old validator at 23:09:51 with the exact legacy timeout, SDK exit 0 and matching result thread 01a1376d-af7c-76e0-973b-94ec566e7a9b. Case sp-requirements-1791930398467051114 had failed with no publication outcomes; clean objective HEAD 6dc6da559a3540ee14b72d125bc2d43ade49b313 remained intact. Supported backend command resume-provider-artifact-once with --validation-replay passed preview and execution. Same task was claimed at 23:11:16 by the repaired 3b0f worker; same SDK thread/result/exit were preserved, no new SDK implementation was started. Runtime HEAD was verified as 6dc6da5. The validator is using the persistent private Git metadata NuGet/Dotnet paths. A bounded network check returned NuGet HTTP 200; caches were growing during restore. Delivery and issue closure remain unverified.
The first replay reached its 600-second restore deadline; the worker recorded typed validation_failed outcomes instead of aborting with an empty case, and the owned restore PID 314 was observed absent afterward. Source, task and original SDK result remained intact. A private restricted snapshot preserves the full task and case before the second replay; cumulative SDK data were not reset.
Actual worker traces established that HTTPS package download succeeded while certificate-related HTTP connections remained in SYN_SENT under the namespace's existing HTTPS-only egress policy. The original SDK stream showed its reported passing test used NUGET_CERT_REVOCATION_MODE=offline. Root did not repeat that workaround or weaken verification. A bounded administrator operation retrieved the original packages' public DigiCert, Microsoft and historical Symantec revocation lists using the controller's existing HTTP access. Each list was independently checked for its issuer's RSA signature and current lastUpdate/nextUpdate before atomic installation into this original task's private .NET CRL cache. Sixteen current lists were installed; hashes, origins and expiries are retained in the restricted task evidence. No network-policy, trust-root, worker-configuration or source change was made. Online verification remains unchanged. This task-specific cache supply is recovery assistance, not proof of automatic self healing.
With materially changed caches, the supported --validation-replay preview and execution accepted the same original task again. It was claimed at 23:22:08 with its same original SDK thread and exit result. Restore finished, assets now reference the persistent Git metadata package folder instead of deleted scratch paths, and the real RepositoryRequirementsTests command is running. No new planner or SDK implementation was launched. Independent test success, publication, delivery and issue closure remain pending.
The second validation replay passed restore (12.78 seconds), the actual catalog-test command (exit 0) and git diff --check. AutoCoder published preserved source 6dc6da559a3540ee14b72d125bc2d43ade49b313 in TheWell Backend DEV PR 6800 and merged it itself at 23:27:32 into DEV 664a6f4bb1a7607aa0f8a5805a93f06defa83b86. Administrator intervention supplied recovery and current signed certificate-revocation cache data; product push and merge were performed by the engine. This is an assisted engine delivery of a catalog-contract fix, not MCP feature activation or completed TEST acceptance.
AutoCoder opened TEST PR 6801. Its exact source 0cac296886745cdd86d936e19c48a81c8488e982 and exact TEST target 5b96d2ce10a1ae0a9752e6424c36503d4324feb9 each contain 95 distinct requirement rows (verified from immutable Azure files). DEV retains 96; these are different scoped catalog revisions. TEST PR 6801 conflicted. The engine queued its original case's bounded merge recovery and actually claimed it at 23:33:19.786228 on the repaired 3b0f worker, model gpt-6.1-sol, SDK thread 01a1379d-1cdf-7022-b24b-29131c927e07. SDK reports preserving original TEST records and passing static checks and restore; compilation/test remains active. No worker was restarted or superseded by Root.
A separate evidenced defect held this continuation under the new-outcome 15-minute admission gate. Planner and implementation already belonged to the original REQ-0001 runtime outcome admitted October 10; the delivery recovery instead received a new change-set-root runtime identity. This splits admission and accounting. The exact source and memberships are retained in the private checkpoint for a regression-tested lineage repair after the current delivery. No live accounting migration or source patch has been applied. All issue closures and the full Goal remain unverified.
AutoCoder's bounded TEST conflict recovery completed successfully and atomically published clean 2078fd4d1a89379e27458e111802fb334548b48f with both captured revisions as direct parents. SDK reported 61 .NET and 93 Python checks passed; independent worker validation passed before publication. Existing TEST PR 6801 was merged by the engine at 23:55:58 into TEST f3dfa5299399b8c37b879ce361505bb5dc7c2cfd. Gitea TheWell Backend worker build run 50/native task20018/job21364 actually passed (both native statuses1) for that exact TEST revision.
The existing artifact relay was repeatedly refusing all new TEST deliveries because Azure run26396 remained locally failed, although its exact TEST source7c32ad3654a304c3688aa19a513ff75cd4364e8a and real Deploy stage had succeeded. It had adopted an already completed run before its own attachment state was recorded. The retained Azure local-worker artifact was a legitimate PipelineArtifact from an independent worker build; its archive hashes differ from the separately observed Gitea bundle, so evidence keeps each producer's receipt separate. The repair verifies the existing bundle against its own retained hashes and exact source, and the actual deployment stage, rather than rewriting or fabricating a common artifact identity.
ProfileBackups PR30 source03fabc7936e23113657c8998d36cfd120222fc79 merged as6a575662b438d67f7ddd605dbdf8da947f715324. Nine worker-01 regression tests passed; the old source failed the actual completed-run adoption check. A read-only live preview verified old deployment and archive integrity, preserved live state, and proposed superseded for the now advanced TEST head. The reviewed source was copied to its canonical host source and installed relay after checking old hashes; canonical and installed hashbeb27178ce5f1564866ad25459ac735701bf07a4094c1f1e0c3553a27d821fed match. Original source is privately preserved for rollback. No identity, permission, timer, production-product or runtime-flag changes occurred. Signed artifact downloads use the existing provider read capability and do not forward its credential to artifact endpoints.
The existing timer then independently preserved the false failure and verified prior deployment in its append-only events, queued exact Azure TEST run26545/pipeline394 for f3dfa5299399b8c37b879ce361505bb5dc7c2cfd, and attached its verified worker bundle. Relay service completed successfully; current state attached. Current TEST application deployment and acceptance are pending. The pipeline wait will be registered and the turn ended; all original Goal issues and requirements remain unfinished, with zero verified native issue closures. Paused pilot generation21 and other preserved stops remain untouched.
Azure TEST26545 succeeded for f3dfa5299399b8c37b879ce361505bb5dc7c2cfd. Actual Deploy and Intake stages succeeded; optional observation/repair stages were skipped. Original case sp-requirements-1791930398467051114 and original implementation task are completed with original SDK thread/result/exit intact. AutoCoder merged finalization PR6802 into DEV and PR6803 into TEST; transient merge-pending errors cleared normally. Authoritative DEV and TEST catalog reads both show REQ-0001 fully_implemented_complete and passed evidence. DEV has 96 records and scoped TEST has 95. This is one completed catalog-contract requirement, with administrator recovery/cache assistance followed by engine-owned publication, conflict recovery, merges, deployment and fulfillment. It does not activate MCP REQ-0095 or close a native issue.
Critical native issue1491 remains unverified. Current SDK observation projection strips historical null fields and sums unknown token counts as zero. Archived original task/cycle frames remain present in autocoder_delivery_prerequisite_resolutions. Scope-bound nonsecret projection and actual decision-workflow verification remain necessary. No follow-up patch was applied here. Original Goal is unfinished; verified native issue closures remain zero.
Original cycle 139928 was delivery_blocked because its immutable review used a prose acceptance target. Original independent review task, candidate b2bbe0898807fe215c601637ca707304fdadaa9b and complete outcomes remain unchanged. Independently verified Gitea PR 2420 merge a8eb3443b74415834b1ab4972b10629b03a816ca, run 9933 (number 3355), task 19893/job 21236, native successful states and attempt 1, artifact 5423 bytes/hash/retention, both Git ancestry checks, and current TEST deployment image/config/UID/generation/readiness.
The existing Kubernetes-authenticated import-managed-test-observation command passed preview and executed its guarded store transition. Cycle is now awaiting_delivery with TEST receipt sha256:fdaf333b65e912f01d67d06108712f3fa184d467b8dfead522fdd08e4182d751. No issue closure, gameplay acceptance, production delivery or paid gameplay authorization is claimed. TEST provider egress remains disabled. A prevention patch and three behavior regressions are saved in codex/governed-acceptance-targets-20261014; compilation is pending, not deployed. #1491 remains unresolved.
Ordinary AutoCoder reconciliation started original live-acceptance task self-improvement-c139928-live-acceptance-a8eb3443b74415834b1ab4972b10629b03a816ca; verified running on gpt-6.1-sol. The recurrence guard is committed as 1f4b1d42550821e944c6cf168f31a7e90cc0ecfc in AutoCoder-Engine PR 12933 into dev. Worker formatting, 4 relevant behavior/retained-review tests and native binary check passed. Exact source reviewed with COMMENT; candidate CI run 4806/native 10017 is running. This patch is not yet integrated or deployed. Native issue closures verified: zero; #1491 unresolved.
PR12933 source 1f4b1d42550821e944c6cf168f31a7e90cc0ecfc integrated into dev 78bda71d1ee2296f688ecd72afe6af2551ed8c0a. Native branch CI run4806/task20021/job21367 SUCCESS with all 3 new regressions executed, 561 library and 1099 binary tests passed, zero failures (ignored cases separate). Candidate run4808/native10019 candidate/finalize/integrate_main/deploy_elected native jobs all SUCCESS. Automatic promotion PR12946 merged main 67e9deb79a8963ff975c208d690d4a89a2264974. Both production and TEST engine deployments observed ready on sha256:253dac72b2b51f9e21a8b14adb32f054e4e49953f495b8748df394de04450790. Main promotion pipeline4810 is still running at this checkpoint.
Original #2968/cycle139928 live acceptance executed and failed (exit1/blocked). Result retained exact original identities, full outcomes and TEST artifact. Worker observed readyz200 but deployment-read403 and browser ERR_CERT_AUTHORITY_INVALID; no authorised gameplay route exists and disabled TEST provider remains protected. Cycle now review_required for missing runtime capability/evidence. No issue closure is claimed. Next engine repair must address scoped observation capability without lending administrator credentials or expanding permissions; identical failed acceptance must not be rerun. #1491 remains unresolved; native closures verified zero.
Production promotion run4810/native10021/job21374 succeeded for main67e9deb79a8963ff975c208d690d4a89a2264974. The subsequent #2968 acceptance failure exposed a concrete visibility gap: current acceptance context serialized only a managed observation fingerprint while skipping the independently validated current workload facts. PR12956 source15b6b103f0e5f3cc5fe6d8df53cb4af180cdd96a adds a compact deployment-only observation to initial prompts and existing task-scoped evidence refresh. Exact case/repository/revision/receipt/fingerprint remain checked, original timestamp retained, future/24-hour-expired facts marked unavailable, and raw sources/full reviews/arbitrary metadata stay excluded. No RBAC expansion, administrator credential lending or gameplay spending authorization.
Worker formatting, eight observation unit tests (four new safety regressions), two isolated PostgreSQL import/replay-context checks and native binary check passed. Source review recorded as COMMENT. Branch CI4811/native10022 running; repair not yet integrated/deployed. Owned test containers removed. #1491 unchanged/unresolved; #2968 behaviour acceptance still unverified; zero native closures claimed.
PR12956 source15b6b103f0e5f3cc5fe6d8df53cb4af180cdd96a integrated into devdac81ea0c61bbb56a6807f0ad1da4d15f770fb00. Native branch4811/task20029/job21375 succeeded with all four new regressions executed (565 library/1099 binary tests passed). Candidate4813/native10026 all four jobs succeeded. Auto promotion PR12978 merged maince1d23eba9f7f11444a038ffe535bf177ea6398d; production promotion4815/native10028/task20038/job21384 succeeded. Production engine observed ready1, generation/observedGeneration2757, source annotationdac81 and image sha256:d895177c4e877081536c34e4cf5ac004ad04b91852bafc9e05c6cad079739c35.
Dispatched existing publish-worker-image.yaml with ref main and exact source_revision dac81 after confirming current main and absence of a duplicate publication. Run4816/native10029 is running; existing reviewed script preserves old worker image and performs guarded publication/rollback. Worker-path observation proof is pending publication, not yet claimed. #2968 remains unverified and #1491 unresolved; no issue closures claimed.
Worker publication4816/native10029/task20039/job21385 succeeded. All seven ordinary workers observed ready on sha256:226830199a67fa31ddeae68a79e51b121454f9a94d37ef20dcc5be2785ca4375, worker01; pilot retained old2ee image/paused scope. Actual original #2968 task-scoped current_verification_observations MCP process on its original worker returned current compact administrator deployment facts, matching case/revision/receipt/fingerprint, with all10original outcomes and no raw sources/full reviews. The read-only probe used the worker's existing evidence connection and no provider credentials/inference.
Original published Chromium reproduced ERR_CERT_AUTHORITY_INVALID. PR13033 source09dde455e89981805afa2f5d9cf811f07a66294d prepares private NSS anchors from the existing TLS bundle, handles IPA labels, uses selected native acceptance scratch as HOME, and shortens only that task scratch to avoid Unix-socket path overflow. Runtime certutil is included; controlled cleanup preserves original work and sibling tasks. Six Python safety checks, native binary cleanup/isolation test, formatting/shell/binary compile checks passed. Real Chromium UID10001 with actual short writable scratch imported123existing public anchors, reached TESTreadyz200 and rejected untrusted HTTPS with certificate verification enabled. A deliberately read-only NSS DB variation failed; the actual existing profile grants only selected scratch writes, which this repair preserves. Browser repairCI4817/native10030 running, not deployed.
Next verified engine queue blocker: native #3057/c142140 is still implementation_queued while its implementation task is blocked after the one automatic planning revision, on an undefined technical native-pause contract. No tracked source changes reported; source/approved handoff unchanged. This active cycle holds the repository lane while valid repair43992 remains accepted without cycle. AutoCoder owns revising the handoff and recovery; no human decision/issue closure inferred. #1491 unresolved; native closures zero.
PR 13108, source f09f06bfc749d48ae029195a995bacf28e64ad63, integrated into dev 8d3dc0dbd21a38135d385a9804dcfdcd5992f705. Branch run4823/native10045, task20058/job21404 passed. Actual logs include 566 library tests, 1100 binary tests, and the isolated PostgreSQL original-task recovery/stop/ownership/replay regression. Candidate run4825/native10048 is pending; this patch is not yet deployed. Original #3057/cycle142140 remains blocked after exhausting its one automatic technical-plan revision, holding the Engine lane. The new guarded command will complete that original interface definition and resume the same provider thread and cumulative runtime outcome; it does not establish issue closure.
Prior browser worker publication run4822/native10035 passed. The installed helper was exercised in the original #2968 acceptance worker under its actual effective UID1240000012: existing public trust bundle imported, TEST readyz returned200, and an untrusted HTTPS certificate remained rejected. No SDK inference or gameplay call occurred. Original #2968 remains review_required; the operator's refusal of paid gameplay remains effective. Native issue closures remain zero.
Candidate4825/native10048 all four native jobs passed. Promotion4827/native10050 task20066/job21412 passed. Production backend ready1, observed generation2761, exact source 8d3dc0dbd21a38135d385a9804dcfdcd5992f705, immutable image 546a663d2002388cc52eda4b2d165113b685fec9f0a010146409e497f4e69276. Main b797abaec21206c2f7feaea6111ab2720d65d0f7 has the identical source tree. Existing repository-managed worker publication dispatched once: run4828/native10051, matching main and source. Prior worker image remains until that publication completes. Original3057 has not been requeued; the worker protocol repair must be present first. No issue closure claimed.
Worker publication4828/native10051 task20067/job21413 passed. All seven ordinary workers are Running/ready on immutable worker image 52dd064ba7587667662a244a707ae861f22da5e1a115d64106a43a6241ac10d0. Scoped pilot retains its old image and paused generation21.
Authenticated resolve-implementation-handoff-once preview and execution succeeded for original cycle142140/task self-improvement-revise-the-approved-plan-to-define-a-concrete-required-acceptanc-c142140-r1. The missing native pause binding and clearance definition now derives the existing authenticated runtime decision records, without adding a pause API, permissions or stop override. The task is queued on its original account/model gpt-6.1-sol/provider thread 01a1375e-9fe0-7ef2-9d71-c7144435369e. Original blocked task and cycle are archived atomically; cumulative runtime remains1550759ms and active with its original172800000ms allowance. Queueing does not prove implementation or issue closure; actual worker execution and original acceptance are next.
The native issue3057 now projects status working, stage implementation. Its original account32561 claimed the same recovered task at05:11:20. The provider reported the old SDK rollout missing, so the existing normal fallback opened replacement thread 01a138d2-9ca8-7d70-81d1-fb49e9e4acfa; the old thread and before-frames remain archived, and the original task/runtime outcome/accounting are retained. The replacement rollout contains the audited technical completion. Actual SDK tool execution has authored src/codex_accounts/operator_acceptance.sql and src/codecartography_native_pause_evidence.sql in the retained worktree. This establishes real execution and source changes, not reviewed implementation, TEST acceptance or closure.
Original3057 was checkpointed and reclaimed after two capacity evidence timeouts while its dedicated automation account remained eligible. The running guard reused the task telemetry connection, allowing queued queries to consume its evidence deadline. PR13152 adds a separate read-only observation connection with the same policy, two-second connection/read limits, five-second polling and fail-closed stop behavior. PR13174 and13187 bind the process/chat fixtures to their isolated schema through startup connection options; stop, process ownership, history and usage assertions remain.
Candidates4831 and4834 failed these fixture checks and did not deploy; skipped dependent jobs are not success evidence. Candidate4837/native10060 passed all four actual jobs20082/21428,20083/21429,20085/21430,20084/21431. Actual logs confirm the busy shared-connection regression plus capacity deadline, early Goal checkpoint, chat storage pressure and before-turn capacity/history regressions passed. Promotion4839/native10062 task20087/job21433 passed. Production backend ready1/gen2763, source 7ed94e160a73ebdbdb9551f53370505e2f53d99a, immutable image 3fd9ee9fad3246a1fe45cdfbada378afc72dd1269affe62daf5daebf39214439. Main 6417059b1767c9904888160f77260d9c0590bdf9 has the identical source tree. Matching worker publication4840/native10063 dispatched once and remains pending. Original3057 is still running; reviewed implementation and original TEST acceptance remain unverified. No issue closure claimed.
Worker publication4840/native10063 task20088/job21434 passed. All seven ordinary workers are ready on immutable image f9272b30c2e47e0d63fc6a77412d8d0c0be8b7e9f8467069d0af53cb8e02f761; pilot21 retains its old image and pause. The busy original worker retained its old image until the provider-window checkpoint. Original3057 was claimed at06:48:02 by eligible automation accounta855 on the repaired image. The exact task, replacement provider thread 01a138d2-9ca8-7d70-81d1-fb49e9e4acfa, retained source and original runtime outcome remain bound; cumulative active runtime is7351505ms. This is live continuation evidence, not original issue acceptance. Original implementation has not reached review or verified TEST delivery. No native issue closure claimed.
Native2292/cycle119154 retained one rejected review at exact commit 5e6b733fdb928832354e50315a3db7545688e284, but its original source branch was absent. The normal Engine recovery refused to proceed without that branch. Verified the original completed verifier, matching repository/cycle/evidence fingerprint, no checkpoint changes, retained rejection and no supervised/pilot/emergency/parked/integration ownership. The commit remained readable. Restored only codecartography/self-improvement/keep-persona-commands-and-initiative-within-authored-role-respon-c119154 at that exact commit using a normal Git push, with no force or history rewrite; verified remote Git and provider API heads. The provider branch-create API returned422 without mutation, so the existing trusted HTTPS Git route performed the restoration. A bounded3.3MiB shallow bare administrator checkout holds the source receipt. Triggered native branch validation3366/global10072. Original review failures and model-backed acceptance remain incomplete; no issue edit, closure, rejection override or paid gameplay occurred. Engine continuation has not yet been observed.
Native3057 also published checkpoint 78d5f8897d672be60190b5a33c2acd3f7638375c; branch validation4845/global10071 is pending. Earlier database validation caught real retained-stop and acceptance-replay defects plus historical schema fixture incompatibilities. The worker continues the original prerequisite/acceptance matrix. No native issue closure claimed.
PRs 13282 and 13304 integrated into development revision 275446e944cfcf6b242756a942e152ac7f134d3c. Candidate 4854 (native run 10081) passed all four actual jobs: candidate 21456, finalize 21457, main integration 21458 and deployment 21459. The deployment is ready at generation 2765, with source revision 275446e9 and immutable backend image sha256:e43b609cdab224999768e46f5bbecd7b3b22105eb0a94b8dc9f1ee9d2c7cf0b5; main is 206064aa92d84ebf7b78cbe5e2914da266739908.
The retained-seed full-slot regression and deterministic concurrent-retry regression passed before integration. The latter fails on the old code and passes after locking admission before rereading the parent and existing child; existing aggregate-budget guards remain enforced. No campaign, provider request, paid gameplay, pilot or account reserve was activated or changed.
Original native issue 2292 remains unresolved. Its reviewed source branch, restored earlier at exact revision 5e6b733, was deleted again at 08:28:18 by the StockFish identity. Gitea's branch tombstone and AutoCoder's repository lifecycle record report it as integrated. The source review and original rejected recovery remain retained, but cleanup currently protects the recovery target rather than its completed-review source branch. The administrator is repairing this source ownership guard before another restoration. Deployment is supporting evidence; no native issue closure is claimed.
Source-preservation PR 13339 and migrated-schema deletion regression PR 13349 integrated as 0b0670e013f4cca8d17a4523dedfb07c99d2beaf. Candidate 4864 (native run 10092) passed all four actual jobs: 21474 candidate, 21475 finalize, 21476 main integration, 21477 deployment. The backend became ready at generation 2766 with source 0b0670e0 and immutable image sha256:53edecee6c9196d479cef1d74f24bd9a8f8f7f4394e5265c11db4c429179fe60. A subsequent normal engine diagnostic integration, development 4381b858328939a269a53d02fc25c3a039b7ea3f via PR 13350, is now ready at generation 2767 and retains the reviewed-source ownership guard. Both store ownership and actual provider-deletion regressions passed before integration.
The administrator restored native issue 2292/cycle 119154's exact source 5e6b733fdb928832354e50315a3db7545688e284 by normal HTTPS Git push, without force or history rewriting. Git and provider API confirmed the head. The branch remains present after subsequent reconciliation. Its rejected review, original failure and acceptance are unchanged. Recovery admission currently waits for EventHorizon issue 3081/cycle 142848, which owns the repository integration lane.
Issue 3081's preserved implementation had already passed independent review e12213d84a26712e43e9c533f6c2deee768e7729, merged via PR 2557 to 398cdddba57c5b43e6c7c3795d3df0bc8a76f2f4, and completed TEST run 3365/native 10046, task 20059/job 21405. The exact receipt artifact 5821 is 2409 bytes and matches the live TEST deployment: namespace event-horizon-test, deployment UID 20d5378f-13e0-48b4-9259-3b236418bd59, generation 13, source 398cddd, image sha256:6227d74cec371c6f1ce900d3a37d5baa63f02e9ea8230021c53225491b100627, configuration hash d68e9366, ready replica 1. Provider attempt 1 was separately confirmed; historical receipt attempt fields remain null.
Authenticated import-managed-test-observation preview and execution retained this existing evidence for cycle 142848 using an exact-hash administrator envelope. This started no deployment or model-provider request and granted no additional worker authority. Normal AutoCoder then queued and claimed original task self-improvement-c142848-live-acceptance-398cdddba57c5b43e6c7c3795d3df0bc8a76f2f4 on included account b556, GPT-6.1 Sol, provider thread 01a13a01-bd84-7bf2-9927-8b7c8e1a1946. Live acceptance is running; original behavior and native issue closure remain unverified. Paid gameplay remains unauthorized, and pilot generation 21 remains paused.
The original EventHorizon cycle 142848 / issue 3081 reached native live acceptance after the supported delivery observation import. That acceptance finished at 10:50 UTC with zero of eight checks passing; TEST gameplay uses the disabled provider and no paid gameplay authority was supplied. It remains unresolved. Issue 1491 also remains failed at live acceptance. Original issue 3057 remains implementation_queued behind the running Engine diagnostic writer; original issue 2292 remains failed pending the retained recovery lane. No native issue closure is claimed.
The live admission ledger showed health assessments at 10:08, 10:23, 10:38, 10:53 and 11:23, plus issue retrospective at 11:08, while an EventHorizon planner remained queued. A requirements bootstrap then started at 11:38. PR 13410, exact source ae8a8a02a90795b783bbc83e08fbed28d338c658, makes new health and retrospective work yield to actually ready current-cycle self-improvement work across repositories/accounts. All current safety checks and the 15-minute spacing remain mandatory; already-admitted maintenance may continue. The regression fails on old source, and all 12 scheduler database tests plus 566 library tests pass on the worker-host disposable fixture. Those two owned fixture containers were removed after saving evidence. CI, reviewed integration, backend/worker publication and original live issue execution remain pending; source/test completion is not recovery.
Candidate 4873/native 10102 has four actual successful jobs. Production backend source 58e50d93ad0ec3ec4591608c4404997c9c279c91 is ready at generation 2769, backend digest eed84a44f9dd9a0c8d9f1315ef64cac8e71ef1ce3000bf992ad2f9187913785a. Worker publication 4876 succeeded from main abe5ed6f25dfb08d151efc2b0bde08cac85778a4 with equal source tree; selected worker digest f411390cdf2e2750d6287f0468eeef43ea860a4f4f57c9830ac743b06374ab45. Six idle ordinary workers were replaced; the busy 3b0 worker remains on its previous image, marked draining with its original issue 3057 implementation running. Pilot generation 21 remains untouched. The diagnostic writer completed at 11:44, and original 3057 resumed afterward with three files changed; this is execution evidence, not issue completion.
Live read-only SQL verified the queued EventHorizon planner has a fresh intrinsically eligible claim proof and codex_engine_health_task_ready=true, but codex_self_improvement_task_ready=false. Its scope is the existing minimal record {cycle_id:144417,repository:EventHorizon/EventHorizon}, without kind. The new check erroneously excluded that actual persisted planner shape. A scoped follow-up permits a missing kind only for the bound current planner with exact repository/cycle, while retaining rejection for foreign cycle/repository and explicit unknown kind. Its worker-host database regression is running. Do not claim scheduling recovery until that change is integrated/published and original execution advances.
PR 13442 exact head 14f8e9daf8c8e68bccca3904ecfead905afd0dfa passed 12 scheduler database regressions on the worker host and branch CI 4878/native10107/job21499. It was reviewed and integrated to dev 4dc330b2653092cdd90fe29564246b388a372b1d; candidate 4880 completed four actual successful jobs. Production is ready at generation 2770 on that source; main 335e48787f5986144bb19c46076ff613028ecf4f has the same tree. Read-only live validation now returns true for both base claim readiness and the current self-improvement cycle readiness on the exact EventHorizon planner, whereas the latter was false before the fix. Matching worker publication has been dispatched. The next admission opportunity is 12:53:20 after a health assessment started at 12:38. A corrected readiness predicate is not proof of an issue claim or delivery.
Original issue 3057 remains running. Its new revision 951e0da8ba3efbbd04099e90300d15c75bfbe9cd failed CI 4877 at native_feedback_tests.rs:583 because a required acceptance decision lacked the presented acceptance_request_id. This is separate from the previous rejected-predecessor guard failure that the revision addressed. Original 2292 remains failed pending the preserved recovery lane. No native issue closure is claimed.
Worker publication 4884/native10113 succeeded from main 335e48787f5986144bb19c46076ff613028ecf4f. Production generation 2771 is ready with selected worker image digest 1d5dab6cc85f2d60f43791c7d3d80eb75ad482265bcbac5cf05e142966d4334b. The live queued health assessment engine-health-a0ba34091e668ab88c8893362f5ef9ecd16401718a02b203ac3c3ae143894d49 has admission blocker 'Governed issue work is ready for execution'; the exact EventHorizon planner's current-cycle readiness returns true. This verifies live maintenance yielding, but the planner is still waiting for the unchanged fifteen-minute window at 12:53:20; original 2292 remains unresolved.
Original 3057 published 326ba50a11aeefaa21b31ee83bbf646a8f14467a, correcting the failed request-binding fixture to require an error and unchanged cycle/case for an unbound required decision. It retains the original implementation and required acceptance protection. Exact branch validation 4881/native10110 is running. The task remains executing; no closure or live acceptance is claimed.
The admitted-start ledger records the exact preserved EventHorizon planner self-improvement-plan-eventhorizon-eventhorizon-1791975137934683579 starting at 13:08:21.992337 UTC after health assessments yielded. It completed planning at 13:14:55 and original feedback cycle 144417 is now implementation_queued with native issue 3155. Its implementation task self-improvement-make-presence-transitions-idempotent-and-give-narration-one-cons-c144417 is actually running on the b556 included account. This verifies issue work proceeds through the repaired scheduler, not merely a readiness predicate. Retained issue 2292 still waits on this repository lane and is unresolved.
The Synchronization requirements retry that took the 12:53 slot completed and reported exact dev requirements delivery 44b41ced91afe07b969977c14aca0208ebe93ef1. This is requirements-file delivery, not implementation of those requirements.
Original 3057 latest exact source fe9beb12113af88cbe4dbce6665f4ca797fe525c passed branch validation 4885/native10114/job21509. Previous 4881 failed when the negative-control suite reused a variant's package test executable on its supposedly restored candidate (0.27-second fresh build retaining the deliberately wrong pending-acceptance projection). The original SDK worker repaired package-artifact invalidation in the test control script; it retained the behavioral assertions and genuine required-human protection. Implementation task still runs while preparing its completion handoff. Independent review, integration/deployment and original live acceptance remain pending. No native issue closure is claimed.
Original task self-improvement-revise-the-approved-plan-to-define-a-concrete-required-acceptanc-c142140-r1 completed successfully at 13:32:03, exit zero, with a clean checkpoint at fe9beb12113af88cbe4dbce6665f4ca797fe525c and successful CI 4885. It returned its implementation for independent review. The engine instead marked cycle 142140 failed: 'Implementation task scope did not match the cycle's approved plan and evidence fingerprint'. Read-only comparison verified that task.plan equals cycle.planning_result, the implementation handoff plan matches, and repository/base/evidence bindings match. The original administrator resolution is in both its stored scope and private autocoder_implementation_handoff_resolutions receipt. The scope reconstruction helper preserves the workspace predecessor but drops administrator_handoff_resolution, making its rebuilt fingerprint differ.
A direct engine repair is under worker-host validation. It reconstructs this context only after comparing the private receipt and deriving the exact supplemented handoff from its archived original; genuine plan/context drift remains rejected. A once-only recovery restores ordinary reconciliation of the same completed task after exact clean source/provider checks, preserving its reply, state, original task/thread and accumulated runtime/usage. It writes the compatibility history to existing feedback case events, does not execute implementation again, and preserves runtime/human/pilot/emergency/ownership/CAS guards. Other queued cases retain the existing claim execution mutex. The first database test stopped on a missing required authoritative_observed_at in the newly inserted disposable feedback fixture; that fixture was corrected and rerun. No live lifecycle SQL writes or issue mutation were performed. Integration, backend publication, the original independent review and TEST acceptance remain pending.
PR 13520 exact head ca54fd809e4475d7ea334a3ffe53b61c5b4d014d passed CI 4886, integrated to dev 6d81ecf7c1a7eaad2d780052eff273d0639dbf98 and completed candidate 4888's four actual successful jobs. Production is ready at generation 2772 on that source. Original 3057 remains failed pending recovery while another Engine cycle 144853 retains the unique repository execution lane. Read-only live validation confirms its original completed clean checkpoint fe9beb12, runtime admission and unpaused scope. No implementation rerun or live lifecycle SQL update was performed.
Engine retrospectives still took 14:23, 14:38 and 14:53 start slots. The normal-queue negative control reproduced the cause: a same-repository priority-ten retrospective makes the planner intrinsically unready before maintenance can yield. Earlier tests used a different repository. PR 13560 fixed that circular priority gate, but candidate 4893 then exposed an overly broad change to legacy acceptance precedence. Candidate job 21534 failed; its other three jobs were native status 4 (skipped), despite the monitor summary calling them successful. No deployment occurred from 4893. The follow-up narrows retrospective deferral to a validated current self-improvement cycle and preserves legacy and already-admitted review vetoes, using one shared SQL ownership validator. All 14 scheduler DB tests and all 3 legacy admission tests now pass; the legacy group is added to the focused gate.
Original EventHorizon issue 3155 / cycle144417 passed independent review, merged through dev PR2634 at 89e04443d733f10add320f3997c3348a362156f3 and completed TEST pipeline3382/native10129/task20178/job21524. Its review/merged ancestry is proven. This is administrator-assisted delivery evidence, not unaided receipt ingestion or acceptance success. No paid gameplay, model-provider configuration, SDK credential lending or production feature deployment was enabled. Live acceptance result remains pending; no native issue closure is claimed.
PR13569 exact head 777e90a4c91bbe96b3266c419e62599c980c76a0 passed branch CI4894/native10137/job21538, integrated to dev32f81724a8108bb67831e4642a3544f06a057e75 and passed all four actual candidate4896 jobs. Production backend and TEST are ready on that source. Worker publication4899 from main88bac6ebafc0fd1c4f14534f6884e287a85de349 succeeded; generation2774 is ready with worker digest380238936293dcc1c5c563fd900da449b06766925d08e6abca20f4f570400b35. Idle workers rotated; busy b556 was preserved on its previous image until its checkpoint. Pilot generation21 remains unchanged.
The live start ledger now records the previously starved Engine planner self-improvement-plan-autocoder-autocoder-engine-1791984755611954556 at16:08:33.624828. It ran on A855/gpt-6.1-sol and completed at16:17:10.828191. Cycle144853 is implementation_queued with native issue3176 and task self-improvement-complete-evidence-backed-recovery-of-exhausted-technical-handoff-c144853; its selected scope is preserved recovery of exhausted technical handoffs, including cycle97076. This is actual scheduling/execution advancement, not a closed issue. Original3057 remains failed with preserved completed code while this other cycle owns the repository lane; the deployed once-only review recovery has not yet admitted it. Original2292 remains unresolved.
Issue3155's original live acceptance completed at15:27. Exact deployment identity passed, but all five behavior outcomes remained capability_unavailable: normal gameplay is required and governed TEST has no separately authorized model access. The existing no-paid-gameplay decision remains effective. Neither TEST delivery nor that acceptance task completion closes3155.
Read-only first-party authentication checks established existing operator API access for both production and TEST as ax_bechju1. The normal TEST escalation inbox rejects unauthenticated requests with401 and accepts the existing operator token with200 (currently zero items). No credential was shared with SDK workers. An initial internal review request returned400 due missing required repository/cohorts parameters; that is a request-shape result, not missing login. No decisions, contacts, issue changes or TEST fixture writes were made. Normal user behavior for original1491 remains unverified; current access supports further administrator-owned verification without a new human login request.
Fresh production records show cycle 144853's initial implementation was superseded at 16:23:52 UTC with another material-gap report and no source changes. Its planner revision completed at 16:39:26 and a revised implementation remains queued. This is not implementation completion or an issue closure. The complete original exhausted gap on correction 138417 and the immutable cycle 97076 startup archive were inspected directly: the archive contains the full 673-byte trace, original worker/claim and checkpoint, attributed producer a442fe6f4543aeb2dc36686383b889fc7f440d83 and trace digest 12ec55a15eb2332c5c1e34d9e17131635e283bf7e36cc0d1ef1260163edc45dd. The worker evidence handoff remains unresolved; repeating planning without those inputs is not recovery.
PR 13620, exact source c2e72c4cf7901d4c2a19c14d31d5ca1600a22745, adds four clearly synthetic operator decisions through the existing TEST fixture startup. It restricts the database to autocoder_test, uses configured existing operator identities, disables the synthetic execution account, preserves answers/delivery history on restart and seeds after issue-store migration. No production decision, issue 1571 pause, worker account or provider permission was changed. All four decision-flow PostgreSQL regressions and 566 library tests passed on bounded worker01 disposable containers; formatting and shell syntax passed. Both owned containers were removed. The branch diagnostic gate now executes this database group whenever the fixtures change; exact branch validation run4900/native10151 is pending. Normal TEST API/UI answer and replay observations remain downstream, and issue1491 remains unresolved. No native issue closure is claimed.
Cycle144853 revised implementation self-improvement-recover-resolved-exhausted-handoffs-through-their-original-corre-c144853-r1 exhausted its automatic material-gap revision with one specific missing decision: legacy administrator receipts hash original stdin bytes, while stored JSONB cannot reconstruct their formatting. Direct source inspection confirmed that the existing receipt validator reconstructs the immutable authenticated admission and exact scope, rather than verifying a canonical JSONB hash. The existing external-administrator repair command was previewed and executed with unchanged task/cycle versions, original change-2 and payload digest 8de89a1671db729c17ea9e80b43a843fdc31c3777d4671d68473391a9d74dc67. The supplement preserves the legacy raw-byte hash as historical transport attribution, requires existing immutable administrator provenance and exact scope reconstruction, and specifies independently recomputable canonical full-payload digests for new versioned controller receipts. Unknown/source-confused inputs fail closed. Required regressions cover differently formatted legacy JSON, forged provenance and controller digest corruption. No automatic retry allowance, runtime authorization, scope, acceptance or operator stop was reset. The same original task subsequently became running at16:53:38 UTC. Current provider thread changed under native worker execution; its previous thread remains archived in the private original-task receipt. No source implementation or issue closure is claimed yet.
PR13620 branch validation4900/native10151/job21555 succeeded, including all four named decision-flow PostgreSQL tests, 566 library and1103 server tests plus existing integration/diagnostic groups. Parent review109 is a COMMENT on exact source c2e72c4cf7901d4c2a19c14d31d5ca1600a22745; it is not independent product acceptance. PR13620 merged into dev6a2e84d6a29fec1fbf84c43b4568866529413fa9. Candidate validation/deployment and normal TEST operator behavior are still pending. Issues1491 and3057 remain unresolved.
This section records investigation, not repair or issue closure. Snapshot times are UTC; the operator's display timezone is Europe/Zurich (UTC+02:00 on this date). The subagent remove_recovery_pr_loop was explicitly paused by the operator over scope concerns. Its source-only worktree and unpublished commits remain preserved. No removal PR, integration, deployment, branch deletion, issue mutation or account change has been performed. The draft removes broader periodic provider mutations and changes REQ-0061; it must not be advanced without resolving that scope concern. Other repair work remains stopped; investigation continues.
Gitea read-only snapshot at 2026-10-14 17:51:28 UTC for AutoCoder/AutoCoder-Engine: 10,781 PRs, of which 9,923 are titled “Review recovered…” and concern just 37 source branches; nine of these recovery PRs are merged. In the preceding 48 hours, 1,867 of 1,963 PRs are these generic recovery entries. This is mainly repeated provider CRUD, not 9,923 independent implementations or model tasks.
The worker table's 48-hour snapshot contains 223 claimed tasks and a summed 27.72 hours of recorded claim-to-finish/current intervals. Health assessments account for approximately 12.2 hours/103 tasks in the earlier same-window sample; native self-improvement implementation accounts for approximately 4.7 hours/14 tasks. These intervals overlap across workers and can undercount historical/resumed attempts; they are not elapsed operator time or verified active computation. All actual_cost_usd_micros records in this sample are null. Tokens (including cached context) and estimated costs are technical/accounting fields, not billed cash or proof of subscription savings.
PR13658, “Review recovered implementation with checkpoint ordering conflict: …c69534”, is one of 409 PRs for the same source branch. All409 have exactly the same body and retained source4a48c5c76ccee0ec84872ae6997d3f7f8df5349d; none is merged. PR13658 is closed. The live lifecycle row retains recovery_validation_failed_review with reason “recovery pull request conflicts with the current default branch”.
The production source src/main/repository_lifecycle_reconciliation.rs performs a periodic15-minute scan. It loads open PRs and indexes only merged closed PRs, disregarding closed unmerged recovery attempts as durable decisions. For an unchanged retained branch without an open PR it emits OpenRecoveryPullRequest and targets repository.default_branch (main for this repository), not the configured dev integration branch. Failed checks or conflicts produce CloseFailedRecoveryPullRequest. Closing does not queue a capable source repair or a concrete human decision. The next scan persists fresh branch observations through record_repository_work_lifecycle_scan, which unconditionally replaces state/evidence, sets active=true and clears resolved_at. The same unchanged branch can therefore become eligible again. Independent sample PRs13628/13648 on branch c65543 carry the same retained sourcef67184887eda90c428901188393652a7fa2253ce; provider status is an unchanged failed validation run2306.
The title template is a generic workflow-state phrase plus a raw branch. Its body supplies only an internal retained revision/state/disclaimer, not the original problem, actual change, validation gap or accountable next action. REQ-0056 requires a semantic approved outcome first; REQ-0061 requires correlated work identity, bounded/idempotent recovery and retained evidence. The template and repeated process do not fulfill those outcomes.
Commit989f515c22a9597b19ba4298b0312044ea4f4fcb on2026-09-01 added automatic orphan-branch recovery PR creation; commit1a11aa83d8787eb86e9b0c9089e39695741ce987 the same day added merge/transplant/close disposition. Their recorded requirement is REQ-0061, “Repository work reaches an explicit verified terminal state”: preserve stranded work and continue it through verified delivery, rather than leaving a hidden backlog. The original human request has not yet been linked; commit authorship and requirement text are not that provenance.
REQ-0061 test evidence dated2026-09-01 marks ORPHAN-RECOVERY and RECOVERY-PR-DRAIN passed after tests and47 created recovery PRs. Reviewed tests cover individual disposition and exact-revision behavior; they do not establish the complete repeated scan–close–scan lifecycle or prevent recreation of a closed unchanged failure.
The broader no-outcome failure was known: cycle93619's retained rationale explicitly records repeated failed lifecycles, substantial attributed cost and zero live-accepted lifecycles. It is documented in documentation/incidents/2026-09-08-worker-acceptance-runtime.md, introduced by0d3c29e0 on2026-09-08. The2026-09-09 system-health decision report (c6e62f17) documents “Issue inventory grows without a demonstrated closure path”, including retained branches and absent demonstrated question-answer-resume delivery. An earlier explicit diagnosis quantifying this exact create–close–recreate PR loop has not been located.
.autocoder/requirements.jsonl, changed autocoder/requirements.jsonl. Repository requirements history contains703 cancelled rows marked “Older blocked task superseded by the current blocked requirement selection”. These counts establish repeated failure/replacement activity; the original prompting/provider path contract needs a selected trace before attributing all197 to one decision. This audit explains dominant repeated work and sampled stage failures; it does not claim every process, historical delivery, root user instruction, provider failure or SuperProject requirement has been exhaustively traced. Billing totals, full cumulative active compute and all original human requests are unavailable in the sampled records. No product result or issue closure is inferred from PR/test/task/merge counts. The engine is still running the existing process, and the authorized removal draft remains explicitly paused. Further process audit stays read-only.
Read-only verification at approximately 18:16 UTC. The removal subagent remains interrupted; its draft has not been pushed or deployed. No managed-product repair, pipeline rerun, issue edit or storage deletion was performed for this audit.
Native change set sp-requirements-1791937721073565028, “Implement REQ-0002: Approved user feedback remains owned by TheWell,” is still awaiting_test_delivery. Its Backend outcome is test_merged, TEST revision c8dfd41a6ed57b955554aced00be9d871402f30f, delivery state failed, recovery task null and recovery attempts zero. The record continues to receive updates, while its progress signature has remained stalled since 13:32:46 UTC.
Azure build 26547 completed unsuccessfully at 01:36 UTC for that exact TEST revision. The failed task was “Stage immutable backend release, roll out and verify TEST,” log 16. Five deployment-script tests passed; the staging pod became ready; the deployment then stopped with: “TEST retention stopped: preserved releases or filesystem reserve leave insufficient capacity.” This proves the failure happened during deployment capacity enforcement. It does not establish which preserved releases may be safely removed, or the present capacity of the filesystem.
The source explains the absence of recovery: reconcile_test_delivery in src/server/codecartography_api_superproject_delivery.rs checks direct_test_pipeline.is_some() for any failed configured route, including mode test_branch. In that branch it records “inspect its run and rerun that pipeline after repair” and does not call queue_failed_test_delivery_recovery. The automatic recovery call exists only in the alternative branch. The configured route for this outcome is definition 394, mode test_branch, stage “Deploy exact build to persistent test.” This failure-message path was introduced in commit cefd9da4126e445c06f4a621c7a6c12b488a848d on September 28.
The requirements scheduler treats every change-set state other than completed/failed/partial/rejected as active and blocking its member. The latest Backend admission record therefore lists 31 runnable requirements as requirements-runner-awaiting-active-change-set, with no newly admitted task. A failing delivery remains represented as an active wait, and later member work remains behind it. This is a verified causal chain, separate from the repeated recovery-PR loop.
The rolling 48-hour task sample contains 224 claims and approximately 28.00 summed claim-hours: health assessments 105 / 12.40 h; native implementation 14 / 5.06 h; error remediation 2 / 2.65 h; native planning 17 / 2.26 h; SuperProject requirements implementation 11 / 1.58 h; verification 6 / 0.89 h; SuperProject planning 10 / 0.84 h; live acceptance 6 / 0.82 h; remaining lanes approximately 1.50 h. Durations are claim-to-finish or claim-to-now, summed across concurrent workers, not measured computation or billed cost.
Health action records in the same window: 1,605 observed, 14 reported, four denied and three executed. These counts do not prove successful repairs; earlier recovery-verification checks found no verified recoveries in this window. Current observations themselves report missing provider-stage evidence, checkpoint identity and runtime prerequisites. In the Backend example, this administrator audit could retrieve the precise failed stage using the existing documented ax_julian Azure PAT route; that does not establish that workers possess the same access.
The current native 48-hour cycle sample is 21 cycles: 16 failed, one implementation queued, one awaiting delivery, two outcomes pending and one review required. Nine failed rows explicitly say superseded before execution; four retain missing live application acceptance, one has implementation scope mismatch, one requires plan revision because prerequisite 43966 remains investigating, and one has an empty failure reason. These categories should not be conflated with 16 attempted implementations failing. Cycle 145151 has completed verification and is awaiting governed delivery; cycle 144853 has a running implementation. Neither is a verified product outcome.
No conclusion that every pipeline always fails follows from these records. The supported conclusion is that repeated bookkeeping, pre-execution supersession, missing acceptance capability, and a deployment failure left without an executing recovery account for different portions of the observed backlog.
Additional read-only traces completed by 18:22 UTC. No execution authorization was renewed, no paid assessment was started, no pipeline rerun was performed, and the removal draft remains paused.
The live canonical operational_priority for native issues 3057 and 3176 contains null important, null urgent and null provenance. Their legacy Boolean columns are false/false and their issue type is bug. Those legacy columns do not prove a priority assessment: OperationalPriority::from_metadata requires attributed canonical decisions. Fourteen of the 199 open native issues have at least one canonical priority decision unset; 176 open native issues have status blocked. This confirms the operator's reported priority defect, not a new discovery attributed to this audit.
Cycle 144853's first implementation, successor planner and current implementation all retain numeric queue priority 200. In continue_self_improvement, a version-2 plan with a single outcome is reduced to only the nested plan, discarding outcome-level important, urgent and rationale before ordinary continuation. The multiple-outcome path instead calls materialize_feedback_outcomes, where feedback_outcome_priority is persisted. Successor tasks also retain their predecessor's queue priority. This explains why planning can label work as a bug and still leave the priority decisions unset.
The current deployed root ConfigMap enables categorization but its authorization expiry is 2026-10-10T23:41:15Z. parse_issue_categorization_authorization returns None after expiry, and main.rs only starts the categorization scheduler when authorization is Some. The latest categorization task started October 10 at 12:10 UTC and ended failed after an HTTP 504. The latest campaign expired October 10 and has admission_exhausted_at set. This is an actual admission stop; merely leaving enabled=true does not keep assessments running. Renewing paid/account-scoped authorization is outside this read-only audit.
Native cycle 145151 is awaiting_delivery, merged revision f56e06353975320c712fb7c1b1d46184e0f6a309, PR 2652. Its verification requires TEST. It has no deployment_handoff, deployment_receipt or managed_test_observation.
Gitea TEST run 3390 (native run 10150, job task 20208) completed successfully for that exact revision. Its retained artifact includes candidate-receipt.json (2,410 bytes). The live event-horizon-test/event-horizon deployment is ready with image gitea.app.lan/eventhorizon/eventhorizon@sha256:2ffdb5f66ac6a1862b462fe2304be5aa1c145bfb19401796391b59d39154b856; its pod-template annotation eventhorizon.app.lan/delivery-source is that same revision.
The exact workflow .gitea/workflows/test-web-delivery.yaml runs on dev push, builds, publishes and deploys TEST, then finalizes and uploads a version-2 candidate artifact. The finalizer binds source SHA, provider run/task/attempt, configuration, migrations and selected integrated persona-edit/frontend checks. It does not publish the compact autocoder/deployment/test commit status consumed by retain_self_improvement_deployment_receipt. The actual commit statuses for this SHA contain only the workflow job's pending/success entries. The continuation sees no expected status and returns None, preserving the generic “awaiting its exact Gitea runner run and immutable receipt” message.
The provider run is terminal, so this is not a verified wait for a live run. It is a mismatch between the successful delivery producer's artifact protocol and the consumer's status protocol. The TEST source annotation and ready deployment prove this candidate reached TEST; they do not prove the requested repeated-item gameplay behavior passed normal-user acceptance. That later acceptance remains a distinct step.
Forty claimed retrospective issue-outcome-review tasks completed within the rolling 48-hour sample. Their prompt expressly forbids changing source, contacting providers, editing issues or triggering work. complete_wrapup calls autocoder_save_wrapup, which saves the assessment and completes its review request; it does not enqueue remaining_work or close the underlying issue. Of the 29 saved assessments in that window, 27 report blocked, one partial and one unknown. Task completion therefore reflects completion of an assessment.
A sampled completed repository-error-remediation task engine-diagnostic-4797b1596f1a58ff-1791965985974100919 reports original recovery still blocked because retained history lacks the original startup-review task and revision. Its cited deployed diagnostic repair preserves identifiers for future failures but cannot establish recovery of that historical work. This is a completed diagnostic command with unresolved original recovery, rather than a silently successful repair.
The four sampled SuperProject monitor commands review Synchronization requirements, save proposal artifacts, and change no tracked source. Three report no findings; one records a warning about temporary review instructions in durable requirements. They are monitoring work, not product implementations.
Native activity in the same window includes 27 development-merged-user-acceptance events and six feedback-accepted events, but no close/completion action among that query's matches. The development-merge event explicitly says TEST validation and authorized delivery continue. Those intermediate events must not be counted as closed issues.
This section consolidates the read-only audit of the engine's observed work lanes. Evidence was refreshed through 18:28 UTC. It completes the investigation of work allocation and the mechanisms explaining the backlog and recovery-PR volume; it does not claim an engine repair, successful issue recovery, or an exhaustive reconstruction of every historical error.
The refreshed rolling 48-hour sample is 224 claimed tasks and approximately 28.24 summed claim-hours:
| Work lane | Claimed tasks | Summed claim-hours | What completion means |
|---|---|---|---|
| Engine health assessment | 105 | 12.44 | An operational assessment returned |
| Native issue implementation | 14 | 5.26 | An implementation command returned; downstream review/delivery/acceptance remain separate |
| Error remediation | 2 | 2.65 | A diagnostic/recovery command returned, possibly with original recovery still blocked |
| Native planning | 17 | 2.26 | A plan or decision returned |
| SuperProject requirements implementation | 11 | 1.58 | Member implementation command returned |
| Native verification | 6 | 0.89 | Independent review returned |
| SuperProject planning | 10 | 0.84 | A change-set plan returned |
| Native live acceptance | 6 | 0.82 | An acceptance report returned; its checks may fail |
| SuperProject TEST merge recovery | 1 | 0.38 | A recovery command returned |
| Retrospective outcome review | 40 | 0.36 | An assessment was saved, with no repair dispatch |
| Member requirements import | 4 | 0.22 | Requirements-file import returned |
| Delivery recovery verification | 2 | 0.20 | A recovery review returned |
| Native delivery recovery | 2 | 0.18 | A delivery recovery command returned |
| SuperProject monitoring | 4 | 0.16 | Drift findings or empty proposal artifacts were saved |
Approximately 44% of summed claim-hours went to health assessment and 24% to the two implementation lanes. Concurrency makes these summed worker durations different from elapsed wall time. The current task rows do not recover all earlier resumed execution intervals. Every task in this 48-hour sample has null actual_cost_usd_micros, so billed monetary spend is not established. PR creation runs in the controller's periodic lifecycle scanner and is not equivalent to one worker claim or one implementation per PR.
The retained native cycle inventory has 966 records: 440 failed, 413 no_changes, 58 delivered, 38 rejected, seven review_required, six outcomes_pending, two review_delivery_pending, one implementation_queued and one awaiting_delivery. A delivered label is recorded lifecycle state, not independently audited normal-user acceptance of all 58 historical outcomes.
Of the 440 failed cycles, 380 have no implementation task, 31 have an implementation task but no verification task, and 29 have a verification task. A verification task's presence does not establish approval. Therefore the dominant failure count represents work failing before source implementation, rather than hundreds of completed implementations failing after deployment.
The largest failure family is 265 requested reviews with no concrete authorized delivery destination. These span Bodylog 228, EventHorizon 20 and Engine 17. Of those, 261 have null selected feedback IDs. Sampled Bodylog plans ask which product behavior should be investigated because no selected feedback was supplied. Source continuation fails when escalation returns no destination or zero deliveries. The current producer now rejects missing authoritative source evidence and suppresses unchanged source decisions; those historical failures do not prove the same empty-input admission remains active today.
Other repeated families include requirements goals blocked (29), pre-execution supersession by newer feedback (23), missing ChatGPT plan metadata (20), invalid MCP transport (10), incomplete review contracts (10), invalid planning affected scope (12 across two signatures), bounded capacity/budget exhaustion, implementation/verification scope mismatches, checkpoint/encoding/database failures and failed provider validation or merge. The remaining distinct reasons were inspected as well: they include actual operator stops, missing exact-revision deployment/acceptance capability, preserved work awaiting inaccessible review, checkpoint identity mismatch and obsolete branch/scope records. Cycle 138417 has an empty reason, so its root cause cannot be established from the cycle's reason field alone.
The 60-day task inventory further distinguishes 197 requirements-import path-contract failures, 703 cancellations of older duplicate blocked requirement selections, and 310 superseded SuperProject implementation attempts. Current cancellation/supersession code updates prior stopped attempts when a replacement exists; these terminal counts alone do not prove those tasks all executed again or discarded their work.
| Stage | Verified mechanism | Consequence |
|---|---|---|
| Prioritization | Single-outcome planning drops outcome-level priority; current categorization authorization expired October 10 | Work can proceed with unset priority decisions and inherited priority 200 |
| Planning and human decisions | Historical plans lacked selected feedback or a real delivery destination | A plan/question is produced without a productive implementation or answer/resume route |
| Requirements import | Historical workers changed autocoder/requirements.jsonl while the contract required .autocoder/requirements.jsonl | Imports fail before product work; current recurrence requires separate present-state proof |
| Implementation/review continuation | Preserved task/plan/evidence identities differ, or a retained workspace/checkpoint cannot be attributed | Work is held rather than safely continued; 3057 remains blocked |
| Provider delivery | TheWell Backend TEST deployment stopped for capacity, and the configured-pipeline failure branch records an error without creating recovery | Failed delivery remains an active member lock, holding 31 Backend requirements |
| Receipt consumption | EventHorizon TEST run 3390 delivers the exact candidate and saves an artifact, while native continuation expects a compact commit-status receipt | Successful TEST deployment remains awaiting_delivery |
| Normal-user acceptance | Required live application capability/evidence is unavailable, including real spending and stop constraints | Build/review/TEST evidence does not prove the requested behavior; original issues stay open |
| Recovery PR reconciliation | Failed closed PRs are excluded from retained merged history; unchanged branches become eligible to reopen against main | Thousands of duplicate recovery PRs with no source repair |
| Health/self-healing | Existing corrections are repeatedly observed/revalidated rather than executed to their original acceptance | Completed assessments accumulate without verified recovery |
| Retrospectives/monitoring | Their contracts save assessments/findings and prohibit implementation or repair dispatch | Completed review commands do not close the underlying work |
At the refreshed Gitea snapshot there are 10,800 Engine PRs, including 9,942 “Review recovered” PRs across 37 branches; only nine of those PRs merged. All 9,942 target main. The c69534 branch behind PR13658 has 410 copies, identical bodies and zero merges. This is a concrete automated create-close-recreate loop, not thousands of independent product fixes.
The health process DID identify the configured-TEST-recovery defect before this administrator trace: finding direct-test-pipeline-failure-has-no-owned-recovery was first recorded October 14 at 01:46 UTC, repeated 36 times through 18:18, and never executed as an action. Feedback 44010 remains accepted, availability_verified=false, with empty linked_work, no cycle and no requirements task. The repair action's existing-feedback branch returns observed/reported and says the correction remains pending; it does not itself implement that correction. In the rolling window, 454 repair_engine proposals include 437 observed, 14 reported and three denied; no health action records recovery_verified=true.
The broad zero-outcome problem was already documented September 8 and September 9, as traced above. An earlier diagnosis of this exact create-close-recreate PR loop has not been located. The requirement's coded purpose and introducing commits are known; the original human instruction that commissioned that particular design has not been established.
Current issues 1491, 2916 and 3057 remain blocked at verification. Issue3176 is working at implementation and still has unset canonical priority. The 21 native cycles created in the rolling 48-hour window contain 16 failed, two outcomes_pending, one review_required, one implementation_queued and one awaiting_delivery; none is delivered. No close/completion activity was found in that window's native lifecycle action check. Successful source work and one verified TEST deployment exist; they do not establish the original requested outcomes or justify counting PRs and completed commands as resolutions.
The common failure is the lack of an executing continuation across several boundaries: evidence or decisions do not become implementable work, failed delivery does not become recovery, successful delivery evidence is not consumed, and repeated assessments do not reach original acceptance. This is an inference supported by the separate causal traces above, not a claim that every command fails or that no historical change ever improved any application.
The removal agent remains interrupted. Its broad local draft is preserved and was not pushed, integrated, tested to completion or deployed. No runtime flag, paid authorization, issue lifecycle or database state was changed during this read-only investigation. Existing production/pilot/payment stops remain in force. The repair task is not completed by this audit.
The operator explicitly requested: “please cancel the subagent and discard its work.” The remove_recovery_pr_loop subagent was interrupted and instructed not to resume. Its exact local worktree /home/ax_julian/temp/autocoder-remove-recovery-pr-loop-20261014 and branch codex/remove-automatic-recovery-pr-loop-20261014 were removed after verifying ownership and unchanged tip da2a5c73db4fe4d1169a65c04cd2bc63318cac69. This discarded its two local draft commits and uncommitted follow-up edits from the active workspace. No remote branch existed. No patch was pushed, integrated or deployed. Earlier statements that this draft is preserved and paused are superseded by this cancellation; the historical audit remains retained.
