The authorized administrator engine repair adds broad operational assessment with task-bound read-only evidence and validated recovery. It preserves existing task identities, accounting, pilot stops, paid-use boundaries and manual reserves. Runtime permissions are not inherited by workers.
PR 10729 and fixture correction 10750 integrated through dev. Candidate Actions run4561 succeeded for source 0b3c2bd200ef66e438f06f3c8770eaad5f0f9e9e; candidate, finalizer, TEST deploy and main integration all actually succeeded. Main revision aede848e30a999c31547039fca54d40bfcedcfd1 has the same source tree. Production promotion run4563 succeeded.
Read-only Kubernetes verification selected context kubernetes-admin@kubernetes, namespace autocoder-engine. Backend and frontend deployments each report one ready replica and source annotation 0b3c2bd200ef66e438f06f3c8770eaad5f0f9e9e. Backend image: sha256:e4e5c6a4e275473b48e8bec1abc305942bd59666d6b5c5c033ade179fe52fe14. Frontend image: sha256:32c9445739dfbf4d818390a2a14aaae6e7792ba0a9aac12ce2d703429a8c9784. Backend service /healthz returned HTTP success with JSON "ok". Historical reconciliation completed during startup; no startup restart was observed.
Worker publication run4564 succeeded through publish-worker-image.yaml, ref main, input source_revision=0b3c2bd200ef66e438f06f3c8770eaad5f0f9e9e. The backend selected sha256:8936169df13fbdbc669577f2f81fd9f89ac7a40cd33c9514db90327b180d778d. All seven regular workers are ready on worker01 with zero restarts; their actual image IDs equal that selection. Native workers have fresh online leases and the exact gpt-6.1-sol catalog entry. The older stopped pilot remains separate. Per-assessment tool registration and evidence-bundle consumption remain unverified because no assessment has executed.
Agent diagnosis and recovery of the original retained EventHorizon cycle119198 are not yet verified. The supervised pilot remains paused at generation21; no override or paid gameplay use is authorized. Latest verified pending count was22 (2queued,18delivery-blocked,2acceptance;0running).
Publication used the existing guarded delivery workflows. Prior backend sha256:f7bfa2059b8c4231f1e73b894f2623a15d92c017979e74319e0f26df4f8ca555, frontend sha256:38e13fe7d746189d2e0d09a087fb24a89ddd4bb64b036bf9b2a1ad16863b8707, and worker image remain identifiable. Schema parking defaults false and preserves original work; no task history or pilot decisions were deleted. Worker publication has an explicit old-image rollback on failure. Pending behavior must remain open even when publication succeeds.
Five included, non-main native accounts pass exact-model lease and preliminary account checks. Ordinary work is denied by provider-window epoch uncertainty: current readings are fresh and report zero usage with future resets, but previously retained higher usage conflicts before the old epoch is proven closed. Repeated raw resets move with observation time; no provider reset redemption occurred today. Initial stale-window diagnosis was superseded by this evidence. Accounting uncertainty, protected reserves and pilot stops remain intact pending authoritative provider investigation. No health assessment or original task advancement is claimed.
Separately, escalation review panics when a native escalation has a nullable superproject identity. That loop is distinct from assessment admission; a scoped source repair is in progress and not yet delivered.
A single read-only native provider quota call confirmed current zero-use windows, future resets and a paid-credit spend-control flag; it did not run a model, refresh credentials or consume a reset. Current REQ-0100 and REQ-0058 allow ordinary dedicated accounts to use fresh included allowance independently of historical accounting settlement, while main/fallback protection remains strict. PR 10792, exact head 91f1b7186d4c919292099e7d82caad1a072e6923, implements that distinction and repairs the separate native escalation review panic. Positive recent provider observations continue to bound checkpoint exposure even while accounting is uncertain. The change preserves all historical epochs, allocations and unsettled commitments; it neither closes an epoch nor redeems capacity.
Independent review found no remaining source blocker after correcting the rate-evidence path. Worker01 validation passed 1,656 native tests, 75 database tests and six pipeline configuration checks, including real claim/running guards and main/fallback rejection. Branch CI run4565/task19615 was running at handoff. Source integration, deployment, actual health assessment execution and original task advancement remain pending. Existing regular workers connect to the store without migrating its schema; backend migration publishes the shared quota predicates.
Branch validation4565 passed for the unchanged reviewed head. PR10792 merged into configured dev as 9e40055961c3df303d083d3d7317c15131fe64ff, with reviewed-head ancestry verified against the remote. Release request4566 succeeded and full immutable candidate4567 was running at handoff; finalization, main integration, deployment and live task behavior remain pending.
Full candidate4567 succeeded for all four candidate/finalizer/TEST deployment/main integration jobs. Production promotion4569 succeeded for main c8e80a14dcf5dff9b39ac3467acd6ffc754b6625. Serving backend and frontend are ready at source 9e40055961c3df303d083d3d7317c15131fe64ff; backend /healthz returned HTTP200 with that revision. Five ordinary native accounts now pass the current-allowance gate. Historical uncertain epochs and main/fallback denials remain unchanged. Backend logs show escalation-history admission with no new panic.
The first actual assessment was claimed by the native worker, then failed before model launch because the task-bound observation gate permits only online while a legitimately executing worker heartbeats busy. The scoped repair will accept busy only for the same running assessment, matching account and admission worker, with scope, stop and freshness checks retained. A subsequent assessment is queued under the existing whole-outcome pacing. No completed diagnosis, governed recovery or original cycle119198 advancement is claimed. The pilot generation21 stop and paid gameplay boundary remain effective.
PR 10814, exact head 0f936ee7be58b2b623b008facb08faec4f956a7d, targets dev. The scoped observation gate accepts online or busy in both status columns because the production heartbeat persists the running status twice; it adds matching account identity and retains running task, scope, admission ownership, desired-online and freshness checks. Worker01 validation passed 1,656 native tests and 13 database tests, including actual claim-to-busy-heartbeat observation and ten invalid ownership/liveness cases. Independent source review passed. Branch Actions4570 is running; dev integration, serving delivery, full worker publication and live assessment/original-work recovery remain pending.
Branch validation4570/task19623 succeeded for unchanged reviewed head 0f936ee7be58b2b623b008facb08faec4f956a7d. PR10814 merged into configured dev as 502c1286f618a621e229f0bc90160255de7f0207 using the exact-head fence; remote ancestry was verified. Release request4571/task19624 is running for that merge. Candidate validation, deployment, full worker publication and actual recovery remain pending.
Full candidate4572 succeeded for source 502c1286f618a621e229f0bc90160255de7f0207; candidate19625, finalizer19626, TEST deployment19627 and main integration19628 all have actual success states. Remote main 6d23e4a5da61ce0d9e071f31c71a4f1f01d66636 contains the reviewed source. Main request4573 succeeded; production promotion4574/task19630 is running for that main revision. Serving production and full worker publication remain separate pending evidence, followed by the same preserved task's recovery.
Promotion4574/task19630 failed on a 15-second production-claim request timeout. The engine reconciled it as terminal before any claim, preserving candidate 502c1286f618a621e229f0bc90160255de7f0207-4572-1; current read-only dependency measurement completed in about two seconds, so no speculative source change was made.
The engine automatically started recovery4575/task19631. A manual retry4576/task19632 was dispatched without a fresh active-run check and overlapped that recovery:4575 was cancelled after its guarded backend patch, and4576 failed HTTP503 while the replacement backend initialized. This operator coordination error is recorded; further manual dispatch was stopped. Backend initialization completed and /healthz now serves source 502c1286f618a621e229f0bc90160255de7f0207. The frontend still serves the previous source pending completion.
The engine started recovery4577/task19633 itself. The cancelled4575 task left Lease autocoder-engine/autocoder-database-migration owned by production-9749-ba1fdd23-4b70-41c1-8991-34fd18ba7699. Read-only evidence proved the exact task terminal, its runner3 container absent, the backend ready and no surviving migration wait. Administrator recovery used the repository ci-migration-lock.sh release for that exact holder; its resource-version check retained other ownership. The lease is now released (resourceVersion32476528), allowing the existing governed recovery to continue. No pilot stop, account reserve, task history or paid-use boundary changed. Full frontend/promotion completion, worker publication and original-work recovery remain pending.
Recovery4577/task19633 failed while its automatic-promotion POST exceeded30 seconds. The engine started4578/task19634 and successfully passed authorization and production claim at17:06 UTC, with exact execution 9752-fb49f5b2-a221-47a1-8839-81dd8a46c1f1. That task owns the candidate again; no additional manual retry was dispatched. No persistent slow dependency is yet proven, so timeout diagnosis remains evidence gathering rather than a speculative source patch. Monitor4578 tracks the same main revision. Backend/frontend rollout and completed production observation must still be verified before full worker publication and original-task recovery.
Recovery4578/task19634 actually succeeded and the candidate is production_live. Backend and frontend are each ready with source annotation 502c1286f618a621e229f0bc90160255de7f0207. Actual backend digest equals selected sha256:83efcf0beb587f1a3c4b9dbc2939afe7bda9cbee43750a97cceb28429a58e87b; frontend equals selected sha256:55029583e3a59785cb90c9f5ee578328e5df9b28641697b3a979429200a052af. External /healthz returned HTTP200 with matching production revision; /pkg/build-revision.txt returned the same source. Production ledger observation and serving checks agree.
After rechecking current main and absence of active worker-publication runs immediately before dispatch, existing publish-worker-image.yaml was dispatched once on main 6d23e4a5da61ce0d9e071f31c71a4f1f01d66636, input source_revision 502c1286f618a621e229f0bc90160255de7f0207. Run4579 is queued. Existing workers still require this compiled evidence-query change; full worker selection, actual assessment completion and original cycle119198 advancement remain pending. The earlier authorization timeout cause was not proven; startup reconciliation delay was observed separately and is retained as a bounded follow-up.
Run4579/task19635/job20975 succeeded (actual Gitea terminal status1). All seven regular worker pods are Ready with zero restarts on the selected immutable worker digest sha256:e864b9eb45b5cb421a0d8222a0805e54420243bfc96a44587b1d3224e0951df0. Backend and frontend remain Ready at source 502c1286f618a621e229f0bc90160255de7f0207.
The next health assessment engine-health-bc72e8796f812a404c1c9479bf20f4d659be94e204456611421e98800998b3cb is queued. Admission explicitly defers new subscription outcomes until 17:34:55.508583 UTC under the existing fifteen-minute cadence. Three native issue reviews have completed; that proves ordinary native execution, not successful health assessment or original-task recovery. The first failed health assessment and its accounting remain preserved. Actual task-bound observation consumption, structured response and validated action remain unverified.
Original snapshot pending22 meant queued2 plus awaiting_merge20, not just two unfinished jobs. At 17:30 UTC the live snapshot is pending26 (queued6 plus awaiting_merge20), running0. Additional health/review jobs account for the changed count. Original cycle119198 acceptance still has its exact-model routing blocker. The pilot remains paused at generation21, with its 2026-10-10 11:32:02 UTC state-change receipt explicitly recording technical-park-repeated-capacity-evidence-timeouts-preserve-same-task. No pilot restart, protected reserve override or paid gameplay was performed.
The 17:35 admission slot went to review1488, which completed17:36:03. Healthbc72 remained queued, now deferred until17:49:59. The stronger source cause is the same-repository priority10 recovery veto in worker_task_claim_methods.rs: it prevents priority80 health assessment while an eligible retrospective review exists. Reviews1418 and1464 have superseded requests but still-queued worker tasks. Claim admission does not check the request state; supersession is only rejected after execution. Thus obsolete work can both consume capacity and veto the diagnostic agent.
New closure reviews were traced to startup status oscillation, not verified card reopening. Issue-store migration normalizes closed cards to completed/cancelled. Lifecycle reconciliation projects their latest no_changes cycles back to blocked while leaving state closed. The native wrap-up trigger treats that blocked-status transition as another closure. Cards113/122 and their deployment-time requests corroborate the chain. Earlier inference that repeated close requests proved reopening was corrected before implementation.
A focused follow-up is being implemented on codex/engine-health-stale-review-admission-20261011, based on reviewed dev502c1286. It preserves terminal card projections, corrects closure/blocked capture, validates review request ownership before claiming, retires only obsolete queued reviews without deleting history or interrupting running reviews, and allows read-only health investigation independently of the queued-recovery repository veto. Tests will use real migrations and retain active-card/case behavior. Source edits are in progress; no new validation, commit, CI or deployment is claimed. Existing cadence, priorities, capacity, stops, task identity and cumulative accounting remain intact.
A superseded request was observed consuming a real admission slot: review1418 completed at17:50:35 despite its request already being superseded. The read-only health agent was then deferred again. Review1490 completed normally with a completed request; legitimate reviews and obsolete reviews are distinguished by durable request state.
The reviewed follow-up preserves terminal closed/cancelled cards during lifecycle projection, captures only genuine close/cancel/nonterminal-block transitions, and retires obsolete queued reviews in the same request transaction and during migration. Existing running/completed task history and accounting remain intact. A shared request/snapshot/revision helper rejects stale review claims in both the Rust claim query and an early database trigger; the early trigger runs before capacity reservation. Valid task-bound read-only health assessments can inspect a repository despite queued continuation, queued recovery, or another account's running repository work. Normal mutating work retains those ownership guards. Account concurrency, model compatibility, reserves, cadence, provider deferral and pilot stops remain enforced.
Independent source review passed. Final worker compilation and both new admission/history database regressions passed; negative claims assert their exact persisted blocker. Full native suite1656 passed, new actual-store restart and native capture regressions passed, and six deployment configuration tests passed. Remaining affected database groups are currently running in the bounded worker01 validation containers. Fixture errors discovered during validation were corrected without dropping constraints or weakening production assertions. The archive has no Git metadata, so the checkout-dependent candidate shell check is deferred to actual CI; no substitute Git state was invented.
At18:12 UTC the live queue was26 pending (six queued, twenty awaiting_merge), running0. Healthbc72 remained deferred until18:20:13 under existing cadence, and original cycle119198 retained its exact-model blocker. No new follow-up commit, CI integration or deployment is claimed at this point; actual health assessment, validated action and preserved-task advancement remain required.
The final worker sweep passed1656 native tests and65 affected database executions; the separate two admission/history tests bring database coverage to67. Six configuration tests passed. All eight changed files matched the tested worker source by SHA256. Independent final review passed and formatting/diff checks passed.
Commit b353c91ae95c621e5fba1749f6435f3e1f109ef8 was pushed on codex/engine-health-stale-review-admission-20261011. PR10886 targets dev. Exact branch validation run4580 is running. Reviewed dev integration, immutable candidate validation, serving publication, full worker publication, actual health assessment/action and preserved original cycle119198 advancement remain pending; no completion or new deployment is claimed.
Branch validation4580/job20976 succeeded. Combined commit status was success, PR10886 remained mergeable with reviewed head b353c91ae95c621e5fba1749f6435f3e1f109ef8 and unchanged base 502c1286f618a621e229f0bc90160255de7f0207. The exact-head-fenced merge returnedHTTP200, with force_merge=false and delete_branch_after_merge=false. Fetched dev ec43261a36fe67edcf34c11cbad2f660732a8655 contains the reviewed commit by ancestry.
Release request4581 is running for that exact dev revision. Full candidate checks, TEST/main integration, scoped production publication and full worker publication remain pending. Current runtime is still the earlier502c1286 source; actual health investigation, validated action and original119198 advancement are not yet proven.
Actual Gitea candidate4582 jobs20978(candidate),20979(finalize_candidate),20980(integrate_main),20981(deploy_elected) all succeeded. Fetched main 687287958ba31503a1e1258db397667ecbabe752 contains reviewed dev source ec43261a36fe67edcf34c11cbad2f660732a8655 by ancestry. Main release request4583 succeeded; existing production promotion4584/job20983 is running. Candidate ledger ec43261a36fe67edcf34c11cbad2f660732a8655-4582-1 is promoting, with matching authorization/execution task19643.
Serving publication, full worker publication and actual health-agent assessment/action remain unverified. No manual overlapping promotion was dispatched. The pilot stop, protected reserve and original-task identity/accounting remain preserved.
Promotion4584/job20983 succeeded. Candidate ledger ec43261a36fe67edcf34c11cbad2f660732a8655-4582-1 is production_live. Backend podautocoder-engine-58dcc4fcf4-6cssg and frontendcodecartography-frontend-57cbd44756-lkrxd are Ready at exact source ec43261a36fe67edcf34c11cbad2f660732a8655. Actual backend digest matches sha256:98819d003e5cbc560df893f01a00b1152a2bc0b453e2b4ad90c63a165f2d77b7; frontend matches sha256:016d6db4ae2068d182cf5bdd8eaa4759e05a21c8004cffdd3f3d79f75597de40. External healthz returnedHTTP200 with matching production revision; frontend build-revision.txt matched.
Backend had one startup restart: previous container exited after account migration exhausted20 retries under SQLSTATE55P03 lock conflicts. The replacement started18:33:35 and is Ready. This is recorded as a bounded startup migration-contention follow-up, not attributed to an unproven query or treated as ongoing failure. Follow-up acceptance would identify the actual blocking transaction and stage timings and prove bounded restart-free startup without interrupting unrelated owners.
Live database now has zero obsolete queued reviews under the installed request/snapshot/revision helper. Current snapshot is twenty awaiting_merge, four queued and one running. Healthbc72 remains queued under the fifteen-minute admission cadence; its actual model assessment/action has not yet occurred.
Immediately before dispatch, current main687287958ba31503a1e1258db397667ecbabe752 and absence of any active worker-publication run were verified. Existing publish-worker-image.yaml was dispatched once with source_revisionec43261a36fe67edcf34c11cbad2f660732a8655, HTTP204. Run4585 is running. Full selected/actual worker image checks and actual original-task recovery remain required; pilot stop and protected reserve are retained.
Run4585/job20984 succeeded. Backend-selected worker image is sha256:368bd057c4dd0fdea1e8c2a6ab8cd9321709cb5196f708f4216d2e0955ccd61a. All seven regular worker pods onworker01 have that exact actual imageID and are Ready with zero restarts. Their fresh online leases corroborate six native exactgpt-6.1-sol catalogs and the openai-compatible source catalog mismatch. Backend restarted as the worker selection changed and is Ready at sourceec43261a, zero restarts on its current pod.
Healthbc72 is preserved and queued; its next normal admission is not before18:50:26.315372UTC. Actual read-only tool use, structured assessment, validated action and original119198 advancement remain unverified. The next check is after that admission window, conditioned on the same task's state and durable receipts. A separate read-only check is verifying that closed cards113/122 remain terminal and do not produce startup closure reviews. No pause, cadence override, paid call or manual task reassignment was performed.
Independent read-only verification found native engine cards113 and122 still closed/completed with workflow_stagecompletion after backend and worker startup. Neither card has matching new wrap-up requests or activity since18:20UTC, covering both startups. The early review-claim guard and request-retirement triggers are enabled. Three current retrospective requests remain queued; no obsolete queued review or running retrospective review was found. Existing completed and superseded histories remain, including completed request1488, current queued1489, superseded1464, and request1492 superseded after its task had already completed. No issue edits or queue changes were performed by this verification.
Health assessment bc72 was claimed18:50:28 and failed18:51:06 before the model or tools started. The Codex app-server could not load task instructions because its enforced filesystem sandbox's bubblewrap helper could not create a namespace. The existing RuntimeDefault OCI seccomp profile demonstrably rejects namespace creation. AppArmor involvement remains unproven. Failed assessment history is retained; no health assessment or recovery action is claimed.
The prepared change selects the already-installed Localhost seccomp/AppArmor profiles for ordinary unfiltered worker containers, as already used by supervised pilot/operator-chat workers. It expands selected containers' namespace and mount syscall permissions; profiles themselves, rootless IDs, dropped capabilities and no privilege escalation remain unchanged. The user explicitly approved this permission extension for all ordinary workers after independent source review and the worker manifest test passed. Actual sandbox allowed/denied behavior and health startup still require post-deployment verification. Existing regular pods mount a service-account token and do not explicitly enforce a read-only container root filesystem; neither contrary property is claimed.
Source is currently uncommitted on codex/ordinary-worker-codex-sandbox-20261011, based on dev ec43261a. Worker01 compilation and the extended manifest test passed; formatting and diff checks passed. Delivery must also ensure existing ordinary pods actually adopt the new profile. Pilot generation21 stop, named health filesystem/network denials, protected manual reserve, original task identity and paid-use boundaries remain effective.
The approved ordinary-worker profile change now includes a manifest generation marker, so existing workers adopt it through the graceful reconciler: active owners drain and remain until their tasks finish; idle stale pods are replaced. All21 worker manifest/reconciliation tests and the health configuration test passed on worker01; formatting/diff checks and independent review passed.
Two bounded diagnostic pods used the selected immutable worker368bd057 image and installed Localhost profiles on worker01, retaining rootless IDs, dropped capabilities and no privilege escalation. Neither mounted service-account tokens/account credentials; no model turn was sent. Actual health app-server thread/start succeeded. Actual command/exec exposed hidden generated helper aliases and missing logical ELF loader aliases under the restrictive profile. The narrow correction exposes only task CODEX_HOME/tmp/arg0 and preserves runtime aliases, with explicit /etc, /nix/store and /run/current-system/sw denials; the masked /etc/ssl allowance was removed. A normal /bin/bash command through the actual app-server passed all nine allow/deny checks: source/scratch access works; repository/outside writes, outside/auth/configuration/protected Git config reads and command network access fail. A positive listener check excluded unavailable-network false evidence. Only synthetic protected sentinels were used. Both temporary probe pods were removed. This establishes infrastructure enforcement, not a completed model assessment.
PR10918, reviewed commit fcf6a481a0724e01cd5e8890bcbce0b2350ee6ef, passed branch validation4586/job20985 and combined status. Exact head/base/mergeability were verified immediately before the fenced merge; force_merge=false and branch deletion=false. Fetched dev7e65c3263e04d73d0dd14292b26b4a2a53fa75b5 contains that commit by ancestry. Request4587 succeeded; full candidate4588 is running. Serving production remains ec43261a with worker368bd057. Full candidate/main/serving delivery, compiled worker publication, actual regular-pod adoption, native health assessment/action and original119198 advancement remain pending. Pilot generation21 pause, historical failures and protected capacity are retained. App artifact attachment was attempted but did not return during its bounded wait; the ordinary PR link remains usable.
Actual candidate4588 jobs20987(candidate),20988(finalize_candidate),20989(integrate_main),20990(deploy_elected) all succeeded. Fetched main e2f32d4dd0f573ff503d057dba2dfdac4b4372df contains reviewed dev7e65c326 by ancestry. Main request4589 succeeded.
Initial promotion4590/job20992 failed at candidate execution reporting after a request timeout, before any production claim or backend replacement. The ledger reconciled the exact terminal task19652 as before-production-claim. Backend remains Ready with zero restarts on its previous pod; authoritative ingress https://codecartography.dev/healthz returnedHTTP200 in0.025seconds. No persistent slow dependency or blocking transaction was established from the bounded checks. The migration Lease is already released (holder absent, resourceVersion32491624); no manual lease mutation or overlapping retry was performed.
The engine itself reserved recovery attempt1 at19:36:52 and submitted promotion4591 for the same main/candidate; the actual provider run is running. A monitor tracks that exact main revision. Serving delivery, full compiled worker publication/profile adoption and native health assessment/action remain pending. Health2b19 is still queued; its creation predates this repair. A read-only audit found that failed assessment fingerprints suppress identical attempts indefinitely and contain no deployed runtime revision. Any required post-repair reevaluation must use verified governed evidence/route, preserving history and cadence; no task reset or fake assessment was performed. Original119198 remains preserved on its incompatible worker, and pilot generation21 stop remains effective.
Recovery4591/job20993 succeeded and candidate7e65c326-4588-1 is production_live, exact execution task19653. Backend podautocoder-engine-c84dfd4ff-b7f9w and frontendcodecartography-frontend-747f4fb9bb-n5hpr are Ready with zero restarts. Deployment annotations, external healthzHTTP200 production revision and frontend build-revision.txt all match source7e65c3263e04d73d0dd14292b26b4a2a53fa75b5.
All seven regular workers are Ready with zero restarts on worker01. Each now has generation namespace-sandbox-v1 and the approved Localhost seccompautocoder/worker-sandbox.json and AppArmorautocoder-worker-sandbox. Rootless/dropALL/no-privilege-escalation settings remain. This proves actual automatic adoption. Their image is still the previous368bd057 digest, so the compiled health filesystem-profile correction still requires full worker publication.
Immediately before dispatch, current main e2f32d4d and absence of active worker-publication runs were verified. Existing publish-worker-image.yaml was dispatched once, HTTP204, refmain and source_revision7e65c326. Run4592 is running and monitored. Health2b19 remains preserved and queued; its next whole-outcome admission is deferred until19:50:50.114569UTC. Actual model assessment/action and original119198 advancement remain unverified. No reserve/cadence/stop override was performed.
The task-owned validation database and sleeping build containers were removed by their exact inspected IDs after validation completed; source commits and test/probe logs remain. The unrelated EventHorizon fixture cache and main conflicted checkout were left intact.
Worker publication4592/job20994 succeeded. Backend selectedsha256:9fbab2dc7f8aa4ef86f5ee7f1b1ac359b2b45ace2e4eb1596cb06d8e0e04b185. All seven actual regular worker imageIDs equal that selection; all are Ready with zero restarts on worker01 and retain namespace-sandbox-v1. Six native workers have fresh online leases and advertise exact gpt-6.1-sol; the compatible-provider source worker does not. The initial catalog audit used wrong id/model keys and was corrected to the actual slug field before any claim of compatibility.
Post-deployment no-model preflight ran inside the actual native worker with a fresh bounded task directory and sanitized app-server environment. Health thread/start succeeded with scratch-only writes and networkAccessfalse. A normal shell command through actual command/exec passed ten checks: source read/scratch write allowed; repository/outside writes, outside/auth/configuration/protected Git config reads, command networking and opening the actually mounted service-account token denied. Token contents were never read. Task-owned preflight files were removed. The unauthenticated diagnostic thread/start attempted an app-server websocket prewarm, rejected401; no turn/start, model inference, login or paid use was requested. This is sandbox/startup evidence, not an actual health-agent assessment. Logautocoder-health-sandbox-token-boundary-probe-20261011.log.
Health2b19 is queued with next whole-outcome admission19:50:50.114569UTC. Earlier failures and original119198 remain preserved. Actual native model/tool use, structured result, validated action and original task advancement still require observation; no completion is claimed.
The repaired serving source and all seven worker images are verified, and the deployed health sandbox passed ten positive/negative checks. No health model assessment or action has yet occurred. Successive new retrospective reviews took the 19:05, 19:20, 19:35 and 19:50 admission slots. At 19:59 UTC there were six queued tasks, twenty awaiting merge and zero running; the next global slot was 20:05:55.606322 UTC. Original cycle119198's exact-model blocker and the gen21 pilot stop remain preserved.
RCA found that the global fifteen-minute cadence serializes starts but selection orders each account locally. The administrator is implementing a narrow preference: only a new retrospective-review outcome yields to an older health assessment that an actual compatible worker has found intrinsically eligible, with current capacity and safety gates rechecked. Started outcomes and all other work retain their existing behavior; no priority promotion, cadence bypass, task reset or pilot resume is authorized by this repair. Isolated branch codex/health-review-admission-20261011 starts at reviewed dev revision7e65c326. Independent source review, worker-host database tests and configured delivery remain required before this scheduling change is accepted.
PR10960 contains commit cd83d2572c99e49725bc9927008a0c493c7ae946 on dev base 7e65c326. Only new retrospective-review outcomes yield to older health assessments with fresh scope-compatible worker evidence and current eligibility. Already-started outcomes, older reviews, fifteen-minute cadence, protected reserves, runtime validation and the pilot stop remain enforced. The readiness lookup observes a fresh database snapshot after the existing claim lock and permits busy workers only when actual account and worker slots remain available. The migration adds one function with no task or history backfill.
Independent source review passed. Worker-host formatting and compilation passed; the initial native unit suite passed 1,657 tests. The final exact five-file snapshot passed all three new database tests plus thirteen related health, review and cadence regressions (16 total). Tests proved cross-account and same-account selection, stale/current readiness and eligibility, real full/spare busy occupancy, unchanged pilot/MaaS ownership guards, continuations, simultaneous admission and first-receipt visibility. Earlier SQL lookup and fixture failures were corrected with existing guards retained.
Branch validation run4593 is running for the exact PR revision. Integration, immutable candidate validation, serving deployment and full compiled-worker publication remain pending. A newer review also took the 20:05 UTC slot and another arrived at 20:08; the original health task and cycle119198 remain preserved and have not advanced. Completion still requires the actual native assessment, independently validated action receipts and progress of the original work through the repaired engine.
Branch validation4593/job20995 passed for cd83d2572c99e49725bc9927008a0c493c7ae946. Final independent review found no remaining blocker. The PR head, current dev base, mergeable state, combined commit status and actual job success were checked before a head-fenced merge with neither force nor branch deletion. PR10960 is integrated into dev revision edcd3b940216ecfa73bc29e4e7cf3a38509d3c66; Git ancestry of the tested commit is verified.
Release request4594 passed. Full immutable candidate run4595 is running for that dev revision. All candidate, finalization, main-integration and elected-deployment jobs must succeed before serving acceptance. Full worker publication is still required because admission selection is compiled into workers. The current serving and worker runtime remains the previously verified sandbox repair; the new scheduling behavior is not deployed yet. Actual health assessment, validated actions and cycle119198 advancement remain mandatory completion evidence.
All four actual candidate4595 jobs succeeded: candidate20997, finalize20998, main integration20999 and elected deployment21000. Main revision 7c084a42aa34dbe3ed5321d74d20d5d03ada82d1 contains reviewed dev source edcd3b940216ecfa73bc29e4e7cf3a38509d3c66; ancestry is verified. Main release request4596 succeeded and production promotion4597/job21002 is running. No overlapping manual promotion was dispatched.
The current serving backend/frontend still use the prior verified sandbox source7e65, and all seven ordinary workers still use image9fbab with Ready=true and zero restarts. Serving verification and full publication of the compiled worker admission change remain required after production promotion. The health task remains queued for the next admission window20:36:08 UTC; the action table is empty and cycle119198 remains awaiting delivery with its original exact-model blocker. Pilot generation21 remains paused. These are pending behavior checks, not evidence of recovery.
Production4597/job21002 succeeded. The ledger records the tested source as production_live. Actual backend and frontend containers are Ready with zero restarts, carry source revision edcd3b940216ecfa73bc29e4e7cf3a38509d3c66, and match selected digests c59d5096… and e1b085e3…. Public /healthz returned HTTP200 with that revision and production environment; the frontend build-revision file matches.
The selected worker image still held the previous compiled claim logic. Immediately after verifying current main 7c084a42 and the absence of an active promotion or worker publication, the existing publish-worker-image workflow was dispatched once with the exact deployed dev source as its input. Worker publication, all seven actual worker images and fresh lease/model evidence still require verification. The health task remains preserved; actual model assessment, validated action receipts and advancement of the original cycle remain acceptance criteria.
Worker publication4598/job21003 succeeded. The selected worker image and all seven ordinary worker container imageIDs match a1952cb202a9a137f541f109345dce2b16abafa023bdef94bcd8cf09fe009e15. All are Ready with zero restarts on worker01 and retain the approved Localhost seccomp/AppArmor profiles and namespace-sandbox-v1 generation. Six native leases are fresh and advertise the exact selected gpt-6.1-sol model; the original compatible-source lease does not.
The new claim logic is active: the preserved health task has a fresh intrinsically-eligible readiness receipt and the queued review admission evidence identifies it as claimable. The fifteen-minute gate still defers new outcomes until20:51:13.713245 UTC. All remaining reviews were created after this health task. The assessment has not yet run and there are no action receipts; original cycle119198 remains waiting on its unchanged model-routing blocker. The next behavioral check is the normal20:51 slot, followed by the actual assessment, validated actions and original-task transition.
After proving exact ownership and that no build or test process remained, the two task-owned worker validation containers were removed. Their source, hashes, validation results, review and delivery evidence remain preserved. Unrelated containers, sessions, historical tasks and the generation21 pilot pause were left intact.
The normal fifteen-minute admission window opened at 20:51:13.713245 UTC. The preserved health assessment engine-health-2b1961a15dc0e6060259d0a55b3b36acc539b277de6b917181a47ab01b4fb478 was actually claimed at 20:51:13.988197 by ordinary native worker autocoder-agent-worker-codex-account-16cc36fa9f3fb675158d21a6fd, using the published a195 worker image and approved profiles. Live thread 01a12cbc-59f8-77f2-953b-bb605c95c142 is running. Its task-owned transcript proves actual native model activity, successful read-only source commands, and calls to the scoped engine_health_observations MCP tool with offsets 20, 40, 60, 80 and 100. This is actual agent execution, beyond preflight/queue receipts. No completed structured assessment or controller action is yet recorded. The original cycle119198 acceptance task remains queued on the unsupported-model worker; its scope/history, pilot pause, account reserves and no paid-gameplay boundary remain preserved. Current snapshot: running1, queued5, awaiting_merge20. Goal remains incomplete pending the assessment, validated actions and original-task progress.
Independent audits proved original119198 completed native implementation and verification on Sept21, with exact selected admission identities, successful results and coherent retained claim timestamps. Both share the active durable outcome with acceptance and predate cost segments. The adopter omits completed legacy claims; a bounded check found36 active outcomes/48 qualifying old stage rows, only this outcome with a queued descendant. A separate review veto ignores the review's actual cadence readiness.
A reviewed four-file patch on codex/legacy-outcome-review-readiness-20261011 (base edcd) adds strict receipt-based historical adoption using genuine original timestamps and the existing table/provenance, plus review-only governor availability. New outcomes retain their cadence; ready reviews, genuine recovery owners, running serialization, stops and capacity remain enforced. Three real DB tests reuse the mandatory subscription CI shard. Source review passed; final designated-worker compilation and DB validation remain pending. Owned bounded worker01 builder/PG have cleanup ownerroot and retention untilOct12 01UTC. The runtime remains edcd/a195, original task queued, and Goal incomplete. No live task/start/policy/priority changes were applied manually.
PR #11012 (https://gitea.app.lan/AutoCoder/AutoCoder-Engine/pulls/11012) integrates reviewed head e99e6b4e26d5f697c99ed29f7fc39acfb9eb83b2 into dev e5aa59f862f3c9840cb7077a601aa076ce459b13. Exact head/base/mergeability and successful branch CI 4599 were fenced before merge; force merge and branch deletion were disabled. Git fetch verified the resulting dev revision and reviewed-head ancestry. The artifact attachment service rejected the Gitea URL as unsupported; this was not an automatic approval rejection.
The worker-host validation completed at 21:20:08: 1,657 native tests plus 17 database tests, 1,674 distinct checks passed, exit zero. Exact final source hashes matched the reviewed source, and actual compiled test listing proved all three new legacy admission tests were present before execution. The initial retained-mtime stale build and a database fixture constraint failure were detected, corrected, independently reviewed and superseded by the final passing run; neither failed run is counted as validation. Evidence: /home/ax_julian/temp/autocoder-legacy-outcome-final-validation-20261011.log. Final new-test SHA256 e0742e6ec707bf4f20f4bcec91f77d9b0e80e326fcc6cb5f5717b0ba495eba0f.
The repair adopts only corroborated successful legacy native claims into the existing subscription outcome ledger, preserving original claim times, task identity and accounting. A queued retrospective review veto now requires that review to be actually available under the existing admission governor. Ready reviews, real repository recovery, runtime and pilot stops, exact-model selection and account reserves remain enforced. Four files changed; the main conflicted working tree and preserved product implementation were not touched.
Live Gitea read-only state at 21:26 UTC: release-request run 4600/id9774 succeeded for e5aa59f862f3c9840cb7077a601aa076ce459b13; full candidate run 4601/id9775 is running for the same source. Register the durable monitor for 4601 and end the turn. Follow-through requires all candidate jobs, production source/digest verification, then full worker image publication because the review-readiness change is compiled into worker code. Before any manual publication, recheck current main and existing active promotion/publication workflows.
Current production remains edcd3b940216ecfa73bc29e4e7cf3a38509d3c66 and ordinary worker digest a1952cb202a9a137f541f109345dce2b16abafa023bdef94bcd8cf09fe009e15; the new repair is integrated, not yet verified deployed. The broad native health assessment and same-task reassignment succeeded, but original task execution and EventHorizon behavior remain unverified. Preserve pilot generation-21 pause, all history, capacity reserves and the paid-gameplay stop. Goal remains active and incomplete.
Owned validation builder 37267e8533af02e0268464af2794b1201daed6a03a0d1045602df54345877351 and PG 87565779310c806fc42564d7421884a4748789bf033aa23f028fa0597cd4de66 are idle, bounded and retained only for candidate follow-through until 2026-10-12T01:00Z; remove only these exact owned containers after final utility, with identity and active-process verification.
Read-only live Gitea receipts confirm all four candidate jobs 21006/21007/21008/21009 succeeded for source e5aa59f862f3c9840cb7077a601aa076ce459b13. Main request 4602 succeeded. Git fetch verifies main 44fe8afd0f42d15ceb2a30fbd5aa808a424bc5ae contains the reviewed dev source. Existing production promotion 4603/id9777/job21011 is running on that main revision; no duplicate promotion dispatched. Register watch for 4603 expected 44fe8afd0f42d15ceb2a30fbd5aa808a424bc5ae and end the turn.
On callback, verify actual production job, release ledger state/source/digests, JSON /healthz revision and frontend build-revision.txt e5aa59f862f3c9840cb7077a601aa076ce459b13. Full worker publication is mandatory for compiled review-readiness changes: fresh current-main and active publication checks immediately before existing publish-worker-image.yaml dispatch, using deployed candidate source e5aa59f862f3c9840cb7077a601aa076ce459b13 as input, expected workflow SHA current main. Preserve original task identity and pilot pause; neither candidate success nor production promotion completes original behavioral acceptance. Owned bounded validation containers remain idle/retained until their final utility.
Production promotion 4603 passed. Public /healthz returned HTTP200 JSON ok, production environment and X-Autocoder-Revision e5aa59f862f3c9840cb7077a601aa076ce459b13; frontend build-revision.txt matches. Candidate ledger is production_live with backend sha256:7a032d8db448967d084f0b0effac6d4efbe81d346f8d22c3604b81ab6e4999a8 and frontend sha256:4b7ae4f9f21939e0ee029782abdd679379820ddbe69d739609cc85ec77811a62. Actual backend/frontend pod imageIDs match those digests, ready, zero restarts.
The original preserved task self-improvement-c119198-live-acceptance-ea849fcb49414d4856aae46fecda345b9468615a is RUNNING, claimed 21:34:46.107437 on native account a8552b49ad4746c1a733eec97dac92c5 / corresponding ordinary worker, thread 01a12ce4-33ec-7961-87e2-114752c66d10. Task completion and actual EventHorizon behavioral acceptance remain unverified.
Compiled worker readiness repair still requires full publication. Read current reconciler and existing tests: active image mismatch drains the worker; idle mismatch replaces it. No direct deletion or interruption of the active task. Rechecked exact current main 44fe8afd0f42d15ceb2a30fbd5aa808a424bc5ae and absence of active promotion/publication immediately before dispatch. Existing publish-worker-image.yaml dispatched ONCE with production candidate source e5aa59f862f3c9840cb7077a601aa076ce459b13, HTTP204. Actual new run 4604/id9778 queued, expected workflow SHA 44fe8afd0f42d15ceb2a30fbd5aa808a424bc5ae. Register durable monitor/end turn; on callback verify selected full image, actual ordinary worker/init digests, fresh leases, active task preserved/completed, and original behavior or genuine external handoff. Pilot generation21 pause, account reserves, history and no-paid-gameplay authority remain unchanged.
Worker publication 4604 succeeded on main 44fe8afd0f42d15ceb2a30fbd5aa808a424bc5ae. Selected immutable full-worker digest sha256:78a75538b6787c410d150e316c0a553e3c2f3874a52abf77e026e5e838d6a4d3 is verified in actual worker AND canonical-instruction init imageIDs of all seven ordinary workers. All seven are ready on worker01, zero restarts, fresh leases with desired_status online. Approved sandbox generation namespace-sandbox-v1 remains present on all seven; container Localhost seccomp autocoder/worker-sandbox.json, Localhost AppArmor autocoder-worker-sandbox, no privilege escalation and dropped ALL capabilities are verified. The active worker was gracefully drained after its retained verification completed, then replaced; a further existing retained verification naturally started at 21:46:19 on the updated 32561 worker. No active session was forcibly interrupted.
Original cycle119198 acceptance task executed normally on the repaired engine from 21:34:46.107437 to 21:38:24.838871, exit zero, matching native thread/result thread 01a12ce4-33ec-7961-87e2-114752c66d10. It performed current application health and session observations, preserved its scope and old admission, and returned explicit evidence_unavailable for the six requested checks rather than substituting fixture/build evidence. The cycle is review_required, integration_owner false, execution_parked false; the product case remains open. Actual broad health assessment completed with 13 findings, 16 native tool calls and validated same-task reassignment. Its separately reported repair feedback43928 is a retained request, not evidence that that separate repair was delivered.
Administrator read-only verification now independently confirms EventHorizon TEST actual ready workload imageID sha256:083c7bd3e08163ddc2f66a72a1c9851ca13b01950624d0a030e5086e81b3432b, matching the original acceptance artifact. Effective deployment override EVENTHORIZON_LLM_API_BASE_URL points to 127.0.0.1; TEST egress admits only PostgreSQL, DNS and existing Qdrant443192.168.1.4. The configured external provider resolves to 192.135.100.187 and is not an allowed model route. No model requests or gameplay were initiated by the administrator.
Independent acceptance audit confirms a genuine external execution/cost handoff for the five gameplay behaviors: operator decision is needed to authorize a TEST model route, gameplay execution and an aggregate spending ceiling; none is currently authorized. AutoCoder retains case296283/cycle119198, original acceptance identity and failed-check evidence for future governed acceptance after material authorized evidence arrives. Current identity proof does not establish gameplay. Do not rerun unchanged acceptance, expand worker namespace access, change the TEST provider or overwrite retained observations to manufacture completion. Existing ManagedTestObservation import intentionally cannot replace the envelope after review_required; exact replay would be a no-op. Current artifact proof is retained separately in this administrator record.
Engine repair acceptance is satisfied through actual native broad diagnosis, validated corrective action, deployed serving/worker/sandbox surfaces and normal execution of original preserved work to this verified external handoff. EventHorizon gameplay behavior is explicitly UNVERIFIED and its product case remains OPEN. Pilot c98913a3-745c-47fb-a104-844acf905a06 remains paused generation21 with unchanged 2026-10-10T11:32:02.788426 timestamp and original capacity-evidence-timeout technical park; no pilot resume occurred.
Queue reconciliation: original pending22 was queued2 plus awaiting_merge20; reporting only2 as total pending was incorrect. Fresh 21:48 observation is queued5 plus awaiting_merge20 (25 pending), with one separate running verification. Raw task states are not all runnable jobs.
Bounded final evidence file /home/ax_julian/temp/autocoder-agentic-health-final-runtime-evidence-20261011.json is 13,061 bytes and contains projected authoritative task/admission/cycle/pilot/queue and runtime observations. Final validation log retains all1,674 checks. After exact container labels/IDs and active-process checks proved no build/test remained, only this repair's builder37267e8533af02e0268464af2794b1201daed6a03a0d1045602df54345877351 and disposable PG87565779310c806fc42564d7421884a4748789bf033aa23f028fa0597cd4de66 were stopped and removed; label-scoped listing confirms none remain. The clean finished PR11012 worktree was removed after reviewed-head ancestry was verified; branch/remote history and evidence remain. Unrelated EventHorizon containers, main conflicted source, pilot and product data were preserved.
The operator challenged the unchanged backlog. Prior completion overstated the broader recovery outcome: execution/admission fixes and a successful native assessment did not reconcile the 18 retained TheWell delivery rows. This repair remains unfinished.
Authoritative stop/resume trace: September 21 user-requested process pause was later followed by October 6 explicit TheWell DEV/TEST resume and October 7 permission for global restart. The 18 row-level September pauses were never migrated. Current source has no consumer of operator_pause_previous_change_set_state, and these rows have no delivery-hold record. The separate generation-21 pilot pause and security/acceptance decisions remain effective. Source conversations were inspected as actual role=user messages; the administrator continuation record preserves exact references.
A further current engine fault is verified in production logs: every 30 seconds, Application feedback queue pass failed: Operational evidence exceeds bounded assessment prompt. This occurs before provider/native feedback admission. The native health agent's existing missing-evidence action only records an observation, so its SuperProject finding had no owned follow-up. Included automation capacity is currently available; the protected main account remains ineligible.
Immediate two-file repair is prepared on codex/health-backlog-recovery-20261012 from dev e5aa59f862f3c9840cb7077a601aa076ce459b13. The initial health prompt uses a bounded navigable projection while preserving the full snapshot/material fingerprint and exact versions through the leased observation tool. An assessment-admission error is isolated from feedback admission; downstream feedback still runs and its own failures remain visible. Independent review passed including real injected-error and oversized-fleet/row regression coverage. Worker formatting and compilation are in progress; no patch is committed or deployed yet.
Fresh Azure evidence distinguishes stale delivery records from current provider state. Terminology and Document Intelligence retained PRs are actually merged; newer pipeline runs have succeeded, though build success alone does not establish current deployment. Datapool's current definition 387 only tests and publishes a Windows artifact; it has no TEST deployment stage. Sixteen legacy conflicting TEST promotions cannot be safely retried as a Linux deployment. All three enrolled direct-TEST route identifiers/stages are unset. Restore original delivery through typed, audited reconciliation and actual target evidence, preserving artifacts/history and a single execution owner for overlapping work.

Register monitor4610/endturn; callback follow actual full candidate and production source/digest verification. Current serving remains previouse5aa source; this repair is integrated, NOT deployed yet.
Delivery-surface correction: source call-site inspection confirms modified prompt() is called only by server queue_engine_health_assessment and modified queue helper is server-only. Existing worker materialization calls unchanged material_fingerprint() and unchanged observation/protocol functions. Therefore this two-file queue repair requires serving backend publication; a redundant full-worker rebuild is not necessary for these changes. Preserve existing ready workers and verify a natural health assessment and provider/native feedback admission after server rollout. A subsequent observation/migration repair affecting worker execution WILL require its appropriate separate worker publication.
