The user requested issue AutoCoder/AutoCoder-Engine#4444 proposing that /mnt/fastdata/tmp/ back /home/ax_julian/temp/, important and not urgent, then withdrew it due to risk and requested a policy-procedure audit. The canonical storage placement policy prohibits temporary work on fastdata/NFS. The assistant-authored issue body incorrectly presented the request as an explicit scoped policy exception, although the user had requested recording an issue rather than resolving the policy conflict.
The issue was closed. The requested filesystem mapping was never performed in this administrator session; inspection found /home/ax_julian/temp still on /dev/sda2 ext4 and /mnt/fastdata on NFS.
The running engine image was gitea.app.lan/autocoder/autocoder-engine:sha-dbc4dd43d6dd, image digest sha256:cec8c20e4ae10b53a19afe5e328997e857579fc0d4f726f857618d379ff3b295. Source inspection used commit dbc4dd43d6dd302c89ed202f250bddd4be8be414, not the different local checkout HEAD.
repository-detached, task ID detached-7-1789495729-slot-2-1789495844-1789495844596882679.AUTOCODER_IMPLEMENTATION_WORKERS_ENABLED value was false. The deployed claim path checks this flag before claiming. This is an execution pause, not policy-conflict enforcement.src/processes/issue_agent/events/issue_opened_standard_prepare.rs selects implementation for bug labels or generated refactors and planning otherwise. documentation/prompts/BUG_ISSUE_OPENED.md states that bug classification grants implementation approval. ISSUE_OPENED.md asks non-bug planning to consider documentation conflicts, but this is not a typed policy authorization gate.At 18:34:40 UTC the administrator cancelled only the exact unclaimed task, using the deployed cancel method's state transition with additional guards for issue identity, queued state and absence of a claim. Its existing record and accounting were retained; state and command_state became cancelled, with withdrawal provenance in error_summary. Subsequent readback confirmed cancelled and never claimed. No worker or automation was started.
The request passed intake and reached labeling. There is no evidence it was blocked because of storage policy. It did not execute; workers were paused and labeling never completed. The eventual label and an implementing agent's policy decision are unknown. A bug classification could have selected implementation; a non-bug classification would have selected planning. Neither proves a reliable policy-conflict gate. Closing the issue alone did not cancel the existing labeling task.
The canonical shared procedure is /etc/codex/instructions/standard_operating_procedures/implement-and-verify-work.md, section Policy conflicts. It distinguishes recording intent from executable authorization, requires concrete conflict disclosure and recognition of already-resolved scoped exceptions, treats issue prose as evidence rather than authority, requires validated state for managed enforcement, and verifies linked task cancellation after withdrawal. It also avoids making explicitly authorized incident repair circularly dependent on its failed documentation service.
Engine enforcement and webhook cancellation defects remain unrepaired by this audit. This record does not authorize resuming paused automation or creating new Gitea issues. No unrelated task state was changed.
Saved and locally committed in the canonical instruction repository as c5de8a5 (no Git remote configured). python3 /etc/codex/instructions/tests/check_instruction_behavior.py passed 12/12 semantic behavior cases, including unresolved policy conflict, an already-resolved bounded exception, recording-only issue creation, and untrusted issue-body approval. These are instruction-level checks, not an end-to-end proof of engine enforcement. git diff --check passed. Final DB readback: zero queued/running tasks and one cancelled task for issue #4444; final Gitea readback: closed. The root temp directory still resolves to /dev/sda2 ext4.
